摘　　要：[目的]探讨胫骨Pilon骨折行手术治疗临床疗效。[方法]对Pilon骨折患者45例，其中35例在伤后8h内行急诊手术；其余10例予以石膏托外固定或跟骨牵引治疗7～14d后，采用切开复位内固定术治疗。[结果]术后2例切口感染和局部皮肤坏死，其余均Ⅰ期愈合。X线片示术后9～36周骨折愈合，平均13周。其中：优18例，良15例，可8例，差4例，优良率为73．3％。[结论]术前认真评估软组织损伤情况，正确选择手术时机，灵活选择内固定方式，是取得良好手术效果的关键。[Objective] To explore the clinical effect of operation on shin Pilon fracture. [Method] For 45 cases of Pilon fracture, 35 were made emergency operation in 8h other 10 were treated with external gesso support and fixation or heel bone traction for 7-14d, then internal fixation of incision and reposition. [Result] 2 cases had cut infection and local skin putrescence, others were cured of phase h X ray showed fracture cure after 9-36w of operation, 13w in average. Among which, 18 were excellent, 15 good, 8 just so so, 4 bad, the excellence rate 73.3 %. [Conclusion] It is the key to get good operation efficacy to earnestly evaluate soft tissue injury, correctly choose operation opportunity, and select proper internal fixation.
