生脉注射液对重度有机磷农药中毒患者心肌损伤的保护作用研究

摘　　要：目的:研究生脉注射液对重度有机磷农药中毒患者心肌损伤的保护作用。方法:将61名重度急性有机磷农药中毒患者随机分为生脉组(34例)和对照组(27例)。生脉组患者静脉滴注生脉注射液80ml,其他治疗两组相同。检测两组患者的CK-MB、cTnT、MYO、收缩压(SBP)、心电图胸导ST段下移之和(∑ST)。结果:生脉组患者的CK-MB、cTnT、MYO、SBP、∑ST分别为(73.3±16.1)IU/L、(0.47±0.08)ng/ml、(78.5±14.5)ng/ml、(100±15)mm Hg、(0.45±0.13)mV;对照组分别为(85.4±19.6)IU/L、(0.72±0.11)ng/ml、(97.7±18.5)ng/ml、(89±16)mmHg、(0.56±0.18)mV;两组间比较差异有统计学意义(P<0.05)。结论:生脉注射液能够减少氧自由基和脂质过氧化产物生成,增加抗氧化酶的活性,减轻心肌能量代谢障碍和超微结构损伤,改善血流动力学状态,从而减轻AOPP患者心肌损伤,提高收缩压,对心肌损伤具有很好保护作用,在重度有机磷农药中毒患者治疗中应该积极推广使用。Objective To study the protective effects of Shengmai injection on myocardial damage of acute organic phosphorus poisoningpatieats. Methods All the 61 patients with severe acute organic phosphorus poisoning were divided into the therapeutic group （n = 34） and the control group （n=27）. The patients in the therapeutic group received Shengmai injection 80ml by intravenous; other therapeutic wa same from the control group. The indexes which must be tested in the 2 groups include CK - MB, cTnT, MYO, SBP, ∑ST. Results The levels of CK - MB,cTnT,MYO,SBP, ∑ST in the the rapeutic group were （73.3 ± 16.1） IU/L, （0.47 ±0.08） ng/ml, （78.5 ± 14.5） ng/ml, （ 100 ± 15 ） mmHg, （0.45± 0.13 ） mV respectively ;and the values in the control group were （ 85.4 ± 19.6） IU/L, （ 0.72 ± 0.11 ） ng/ml, （97.7± 18.5 ） ng/ml, （ 89 ± 16） mmHg, （ 0.56 ± 0.18 ） mV ; there were significant difference in the results between the 2 groups （ P 〈 0.05） o Conclusion Shengmai injection can he used widely in patients with acute organic phosphorus poisoning. It reduces the production of oxygen free radicals and malondialdehyde（ MDA）, increases the activities of antioxidase, alleviates the energy metabolism disease and the ultrastrueture injury of myocardium, improves the hemodynamics, so it can alleviate myocardial damage, improve the systolic pressure. It has better protective effects in patients with myocardial damage.
