Treatment of Thoracolumbar Burst Fracture with Pedicle Screw Internal Fixation and Transpedicular Bone Grafting in Elderly Patients

摘　　要：目的探讨经椎弓根植骨后路内固定治疗老年人胸腰椎爆裂性骨折的临床疗效。方法对21例老年人胸腰椎爆裂性骨折的患者行经椎弓根植骨后路内固定手术,术前、术后及随访时测量伤椎前缘高度与正常高度的比值、后凸Cobb角,了解神经功能改变及腰背疼痛变化。结果术后伤椎前缘高度与正常高度的比值、后凸Cobb角恢复满意。随访12-36个月（平均22.4个月）,无断钉及内固定物松动,伤椎前缘高度与正常高度的比值、后凸Cobb角无明显再丢失,神经功能及腰背疼痛明显改善。结论经椎弓根植骨后路内固定治疗老年人胸腰椎爆裂性骨折可重建脊柱前、中柱的稳定性,防止后期矫正角度及椎体高度的再丢失。Objective To study the clinical efficacy of transpedicular bone grafting combined with pedicle screw internal fixation in the treatment of elderly patients with thoracolumbar burst fracture. Methods 21 elderly patients with thoracolumbar burst fracture were treated using transpedicular bone grafting and pedicle screw internal fixation. The ratio of anterior injured body height to the normal height,the Cobb＇s angle,the neuropathic function and back pain were analysed. Results The operative reduction had an satisfactory effect on the ratio of anterior injured body height to the normal height and the Cobb＇s angle. After a followed-up of 12-36 months（average 22.4 months）,the ratio of anterior injured body height to the normal height and the Cobb＇s angle didn＇t obviously reduce ,and the neuropathic function and back pain improved obviously. There were no fixation failure. Conclusion Transpedicular bone grafting combined with pedicle screw internal fixation can reconstruct the stability of anterior and middle column,prevent the losing of the vertebrae＇s height and later kyphosis in elderly patients.
