摘 要:目的探讨体外受精-胚胎移植(IVF-ET)过程中基础卵泡刺激素(FSH)水平及年龄预测卵巢储备及IVF-ET结局的价值。方法对1999年5月至2006年3月在郑州大学第二附属医院生殖中心行IVF-ET助孕术的376个新鲜周期结局进行回顾性分析,比较不同FSH水平和不同年龄患者卵巢反应性和IVF-ET结局的差异。结果FSH<10 IU/L患者Gn用量明显低于10 IU/L≤FSH≤15 IU/L和FSH>15 IU/L组患者,而取卵数明显高于其他两组(P<0.05=。年龄>35岁患者的基础FSH值、Gn用量显著高于其他两组(P<0.05=,而取卵数、卵裂率、胚胎植入率、临床妊娠率和继续妊娠率均明显低于其他两组(P<0.05=。低龄高基础FSH(年龄<38岁且FSH≥10 IU/L=患者的胚胎植入率、临床妊娠率和继续妊娠率均显著高于高龄低基础FSH(年龄≥38岁且FSH<10 IU/L=患者(P<0.05=。不同FSH水平患者受精率、卵裂率、移植胚胎数(ET数)、胚胎植入率、临床妊娠率、流产率差异均无统计学意义(P>0.05)。结论基础FSH水平可以预测卵巢的储备功能和取卵数目,但不能预测IVF-ET的临床结局,而年龄对IVF-ET的临床结局有一定的预测价值。Objective: To evaluate the impact of basal follicle stimulating hormone (FSH) level and female age on clinical outcome of in vitro fertilization and embryo transfer (IVF -ET) , Methods: 376 consecutive IVF -ET cycles performed in Productive Medicine Center of The Second Affilitated Hospital of Zhengzhou University from May 1999 to March 2006 were analyzed retrospectively. The ovarian response and outcome of IVF - ET were compared in patients with different basal FSH level and different age. Results : In FSH 〈 10IU/L group the gonadotropin doses had less than that in other groups ( P 〈0. 05 = , while the number of retrieved oocytes had more than that in other groups ( P 〈 0. 05 =. The basal FSH level and gonadotropin doses of patients 〉 35 years are more than that in other groups ( P 〈 0. 05 ) , but the number of retrieved oocytes, oocyte cleavage rate, implantation and pregnancy rate are less than that in other groups ( P 〈0. 05) . The patients 〈38 years and FSH≥10IU/L had higher implantation and pregnancy rate than patients ≥38 years and FSH 〈 10IU/L ( P 〈0. 05). The fertilization rate, oocyte cleavage rate, number of embryos transferred, implantation rate, pregnancy and miscarriage rate by different FSH level had no statistical significance ( P 〉 0. 05 ) . Conclusion : The basal FSH can predict ovarian response and the number of oocytes retrievd, but cannot predict the clinical outcome of IVF - ET, while the female age can do.
