低张水灌肠多层螺旋CT诊断乙状结肠癌的研究

摘　　要：目的研究低张水灌肠多层螺旋CT(MSCT)对乙状结肠癌的诊断价值。方法对65例临床怀疑乙状结肠癌的患者行低张水灌肠多层螺旋CT增强扫描并进行图像重建,观察肿瘤情况,作出诊断及分期并与病理对照。结果低张水灌肠乙状结肠充盈显示良好,对乙状结肠肿瘤显示清晰。多层螺旋CT检出全部41例乙状结肠癌,敏感性为100%。依据Dukes分期标准,MSCT分期与手术病理分期相对比,其符合率分别为:A期60%,B期93.3%,C期80%,D期100%,总符合率为92.7%。结论低张水灌肠MSCT能较好的显示乙状结肠癌的形态、大小、周围组织侵犯及远处转移情况,CT分期与手术病理分期有很好的一致性,MSCT是乙状结肠癌术前诊断的重要影像检查方法。Objective To explore the value of multi-spiral CT（MSCT） combined with low-tension enteroclysis in the diagnosis of carcinoma of sigmoid colon. Methods The enhanced image scanning and video rebuilding by water enema multi-spiral CT were performed in 65 suspected cases of sigmoid colon carcinoma, and the diagnosis result and classification were compared to pathologic diagnosis. Results The images of sigmoid colon carcinoma were displayed much clear by low-tension turgid enteroclysis. All 41 cases of sigmoid colon carcinoma patients were detected by muhi-spiral CT（MSCT） scanning,and the sensitivity index reached up to 100%. In the light of Dukes Classification Standards ,the coincidence rate between MSCT classification and operational pathologic classification was as follows：Stage A 60% ;Stage B 93.3% ;Stage C 80% ;Stage D 100% ;the overall matching ratio was 92.7%. Conclusions MSCT with low-tension enteroclysis can clearly show the shapes, size,infiltration and metastasis of the carcinoma. CT classification is highly consistent with operational pathologic classification ; MSCT is an important form of video examining maneuver as regards pre-operation diagnosis on sigmoid colon carcinoma.
