摘 要:目的探讨磁共振扩散加权成像(DWI)评价动物模型的正常淋巴结、淋巴结炎与淋巴结转移的可行性及其作用。资料与方法20只健康新西兰白兔随机分为两组,分别建立淋巴结炎模型和淋巴结转移模型,以炎症组的对侧下肢不作任何处理作为正常对照。测量各组淋巴结的表观扩散系数(ADC),同时在T2WI、DWI图像及病理检查中分别测量各组淋巴结的大小,并进行病理结果分析。结果DWI可清晰显示各组动物模型的腘窝及髂内淋巴结,测量的大小与T2WI及病理检查所测得大小无显著差异(P>0.05)。炎症组和转移组淋巴结的大小无明显差异,两者均大于正常淋巴结。三组淋巴结均表现为为等T1长T2信号,DWI呈高信号。正常淋巴结、淋巴结炎及淋巴结转移的ADC值分别为(1.35±0.15)×10-3mm2/s、(1.14±0.02)×10-3mm2/s和(0.78±0.07)×10-3mm2/s,三组间ADC值差异均有统计学意义(P<0.01)。结论兔模型DWI图像较高;DWI对兔淋巴结炎及淋巴结转移能提供功能信息。Objective To investigate the feasibility of MR diffusion-weighted imaging (DWI) on normal,inflammatory and VX2 carcinoma metastatic lymph nodes in the rabbit model and the potential application in the differential diagnosis. Materials and Methods 20 New-Zealand white rabbits were randomly divided into two groups. Lymphadenitis model and metastatic lymph node model were made. The lymph nodes of the contralateral leg in the lymphadenitis model group acted as normal control. DWI, T1 -weighted and T2-weighted images were performed in all animal models. Apparent diffusion coeffi- cient ( ADC ) value of the lymph nodes was measured in all cases. The size of the lymph nodes on T2 WI, DWI and patholog- ical sections was measured and compared. Pathological study was completed after lymph nodes resection and hematoxylin-eo- sin (HE) staining. Results DWI clearly demonstrated normal,inflammatory and metastatic lymph nodes in animal mod- els. There was no statistical difference in the size of the lymph nodes between measurements on T2WI, DWI and pathological examination( P 〉 0.05 ). There was no statistical difference in the size of the lymph nodes between inflammatory group and metastatic group, but the size of the lymph nodes in inflammatory group and metastatic group was bigger than that of normal group. All lymph nodes appeared as identical T1 long T2 signal intensity and high signal on DWI. The mean ADC value of normal,inflammatory and metastatic nodes was (1.35±0.15)×10^-3mm2/s, (1.14±0.02)×10-3mm2/sand (0.78±0.07)×10^-3mm2/s, respectively. There was statistical difference in the ADC value between three groups (P 〈 0.01 ). Conclusion High quality DWI images could be obtained in rabbit model. DWI could provide physiological and functional information for diagnosis.
