摘 要:目的检测细菌性阴道病(bacterial vaginosis,BV)患者阴道局部白介素-2(interleukine-2,IL-2)、白介素-8(interleukine-8,IL-8)及白介素-13(interleukine-13,IL-13)的变化,探讨它们在疾病及机体免疫防御机制中的作用。方法选取育龄女性细菌性阴道病患者62例(BV组)作为研究组,同期选取健康体检的无阴道炎症的育龄女性66例作为对照组,用ELISA方法检测两组阴道灌洗液IL-2、IL-8、IL-13及免疫球蛋白E(i mmunoglobin E,IgE)的水平。结果①与对照组阴道灌洗液IL-2(10.08pg/ml)、IL-8(2565.98pg/ml)、IL-13(27.84pg/ml)水平相比,BV组阴道灌洗液IL-2(8.64pg/ml)显著下降(P=0.02),IL-8(2964.28pg/ml)无显著性变化(P=0.321),而IL-13(39.17pg/ml)则显著升高(P<0.001);②与对照组相比,BV组阴道灌洗液IL-2/IL-13比值显著性下降(P<0.001);③与对照组IgE水平(0.89I U/ml)相比,BV组(0.93I U/ml)无明显变化(P=0.401)。结论①细菌性阴道病时阴道灌洗液IL-2下降,提示阴道局部细胞免疫功能受到抑制;②阴道灌洗液IL-13升高,不排除与阴道局部体液免疫作用有关;③月经周期对BV的抗感染免疫无影响;④IL-2/IL-13比值下降提示可能出现了Th1/Th2细胞的免疫功能失衡。Objective To investigate the levels ofinterleukine-2 (IL-2), IL-8, IL-13 withincervicovaginal lavage fluid in the women with bacterial vaginosis (BV), and analyze their relationship with the host defenee against BV. Methods Cervicovagial lavage fluid was collected from 62 patients with bacterial vaginosis which is determined by Arosel criteria (BV group), and 66 healthy women (control grouop) at child -hearing age. The concen- trations of IL- 2, IL-8, IL-13 and immunoglobin E (IgE) were measured by enzyme-linked immunosorbent assay (EI.ISA). Results ① Compared to the control group, IL-2 was significantly decreased (10.08 pg/ml vs 8.64 pg/ml P=0.02). IL-8 had no significant change (2 565.98 pg/ml vs 2 964.28 pg/ml, P=0. 321) and IL - 13 was significantly increased (27.84 pg/mlvs39. 17 pg/ml, P〈0.001) in BV group. ② The IL-2/IL-13 inBVgroup was significantly lower than the control group (P〈0.001). ③ Compared to the control group, the level of IgE presented no change in BV group (P=0. 401). Conclusions ① The celluar immune function of vagina may be inhibited in patients with BV. ② The humoral immunity of vaginal mucosa perhaps plays a role in BV patients. ③ Menstrual cycle has no effect on defence of BV.④ The ratio of Th1/Th2 may be inbalance in vaginal mucosa of BV patients.
