药物输注治疗单纯性乳管炎的临床应用

摘　　要：目的:探讨经溢液乳孔输注药物治疗单纯性乳导管炎的临床应用价值。方法:经临床检查、钼靶摄片及乳腺导管造影确诊为单纯性乳导管炎的惠者63例,其中乳导管扩张症52例,慢性乳导管炎11例。37例可触及质地较软的小包块。经溢液乳孔输注庆大霉素8万单位加地塞米松5mg混合液治疗。每周2次,治疗3周为一个疗程。结果:治疗1个疗程后,38例停止溢液,其中31例乳房胀痛消失;23例乳头溢液明显减少,溢液颜色变清。24例经第二个疗程治疗后(3～4次),其中20例溢液停止,18例胀痛消失,2例胀痛减轻;3例反复挤压乳头偶有少量溢液。37例包块经过治疗后30例包块消失,7例包块缩小或变软,2例效果不佳。结论:明确诊断为单纯性乳导管炎后方可行药物输注治疗,效果明显.值得临床推广运用。Objective：To evaluate the effect of drug infusion via the discharging lactiferous ducts in the therapy of simple galactophoritis. Methods：There were 63 patients with simple galactophoritis diagnosed by clinical examination,molybdenum target mammography and galactography,including ductal ectasia （52 patients） and chronic galactophoritis （11 patients）. Small soft lumps could be palpated in 37 patients. Solution of 80000 unit cidomycin mixed with 5mg dexamethasone was infused via the opening of discharging duct for twice a week, and three weeks as a complete course, Results： After one course of treatment,cease of discharging was seen in 38 patients and disappearance of mastalgia in 31 patients, The volume of nipple discharge decreased obviously and its color became clear in 23 patients, 24 patients undertook the second course of treatment,cease of discharging was seen in 20 patients;disappearance of mastalgia in 18 patients and improvement of mastalgia in 2 patients. In 3 patients,small volume of discharge could be seen after the nipple was repeatedly squeezed, Of the 37 patients had breast lump before,30 patients had the lumps disappeared and 7 patients had the lumps softened or shrunken, Poor therapeutic effect was showed in 2 patients, Conclusion,, The treatment of drug infusion should be applied in patients with the definite diagnosis of simple galactophoritis, the treatment is simple, effective and worth recommending.
