摘 要:目的研究Bcl-2(B细胞淋巴瘤/白血病基因-2蛋白)和FasL(体内Fas蛋白的天然配体)基因与子宫内膜异位症(EMS)之间的关系。方法采用免疫组化法(SP法)来检测Bcl-2与FasL在正常子宫内膜(19例),异位内膜(33例)及在位内膜(11例)中的表达。结果①正常内膜组:Bcl-2与FasL主要表达于细胞浆及细胞膜上,Bcl-2在增生期的表达显著高于分泌期(P<0.05),FasL在增生期的表达显著低于分泌期(P<0.05),即Bcl-2及FasL在正常子宫内膜细胞中的表达有明显周期性。②异位内膜组:Bcl-2及FasL的表达同样位于细胞浆或细胞膜,分泌期的表达与增生期相似(P>0.05),因此在异位子宫内膜中的Bcl-2与FasL呈持续表达且丧失周期性,且二者在异位内膜中的表达显著高于正常内膜(P<0.0001)。结论以上结果提示具有抗凋亡活性的子宫内膜细胞逆流至盆腹腔后,因其高水平Bcl-2及FasL表达,得以逃避机体免疫清除系统的影响而在异位存活、种植,为子宫内膜异位症的发生创造了先决条件。Objective To research whether the endometriosis is associated with Bel-2 and FasL gene. Methods Immunohisto- chemistry method (streptavidin - peroxidase method) was used to detect the expression of Bel-2 and FasL gene in 19 specimens of normal endometrium tissue, 33 specimens of endometriosis tissue and eutopie endometrium from patients with endometriosis (11 eases) .Results The control group:the expression of Bel-2 and FasL predominated in the glandular cytoplasm and/ or on the plasma membrane .The expressions of Bel-2 in proliferative phase was higher than that in secretory phase signifieantly ( P 〈 0.05) .The expressions of FasL in proliferative phase was less than that in secretory phase signifieantly ( P 〈 0.05). So the expressions of Bel-2 and FasL obviously cyclic changes throughtout the menstrual cycle in nomal endometrium tissue.2. The endometriosis group: The expressions of Bel-2 and FasL also predominated in the glandular cytoplasm and/or on the plasma membrane. Both Bel-2 and FasL expression in proliferative phase was the same as secretory phase ( P 〉 0.05). So Bel-2 and FasL expression in adenomyosis had no cyclic change and their staining had remained in the whole menstrual cycle. The expressions of Bel-2 and FasL in endometriosis was higher than that in normal endometrium signifieandy( P 〈 0.000 1 ). Conclusion It is suggested that the retrograding of these active antiapoptotie endometrial cells may play a key role in the pathogenesis of endometriosis.
