阴道超声结合尿LH试纸预测排卵在不孕症中的临床应用

摘　　要：目的探讨阴道超声(TVB)结合尿LH(黄体生成素)试纸预测不孕妇女的卵泡发育、排卵时间及促/超排卵时机掌握的临床意义。方法阴道超声结合尿LH试纸测定108个自然月经周期(A组),75个CC周期(B组)、42个HMG周期(C组)的卵泡生长、尿LH峰、排卵、子宫内膜情况。比较3组卵泡生长速度、优势卵泡数、排卵时间、过早LH峰和LUF(黄素化未破裂卵泡)发生率、尿LH试纸预测排卵的准确性。结果比较3组卵泡生长速度,B组与C组明显快于A组(P(0.01)。优势卵泡数:B组与C组的优势卵泡数均高于A组(P(0.01)。尿LH试纸测定阳性结果的排卵率91.26%(188?206)。C组排卵日子宫内膜厚度高于A组和B组(P(0.001)。C组LUF周期发生率16.67%,高于A组和B组(P(0.01)。B组与C组周期过早LH峰发生率12.8%。尿LH峰后100%在48h内排卵。结论阴道超声联合尿LH试纸测定对监测卵泡生长及预测排卵是一个简单易行、准确可靠的方法。连续的尿LH试纸测定更能动态的反映卵泡生长并准确预测排卵时间。Objective： To investigate the clinical value of transvaginal ultrasonography combined with LH urine test paper （ Urinary luteinizing hormone ） in predicting follicular development, the period of ovulation and superovulation in the diagnosis and therapy of infertility. Methods： Use Ransvaginal ultrasonography combined with LH urine test paper to measure the growth of ovarian follicle, the urinary LH peak, ovalution and endometrium in 3 groups which are group A （108 natural menses cycle）, group B （75 CC cycle）, group C （42 HMG cycle）. Compare the growth velocity of ovarian follicle , number of dominant follicle, period of ovalution, LH peak, incidence rate of LUF and reliability of predicting ovalution with LH urine test paper amount 3 groups, Results ： Growth velocity of ovarian follicle ： B, C is much quicker than A （ P 〈 0. 01 ）, Number of dominant follicle ： B, C is much more than A （ P 〈 0. 01 ）. The positive rate of LH urine test paper is 91.26% （188/206）. Compared with A and B, the thickness of endometrium during ovalntion of C is obviously higher （ P 〈 0. 001 ）. LUF＇s incidence rate of C is 16. 67% , higher than A, B （ P 〈 0. 01 ）. The incidence rate of LH peak is 12. 8%. There is an 100% incidence of ovulation within 48 hours after the urinary LH peak. Conclusion： Ransvaginal uhrasonography combined with LH urine test paper is a convenient, precise and reliable method which can monitor the growth of ovarian follicle and predict the ovalution. Continuous LH urine test paper can grasp the growth of ovarian follicle more dymanic and predict ovalution accurately.
