摘 要:目的探讨输注新鲜冰冻血浆对早产极低出生体质量儿颅内出血的预防效果。方法选取中山大学附属东华医院出生的早产极低出生体质量儿为研究对象,随机分为实验组29例和对照组32例,对照组沿用常规方法预防颅内出血,实验组在其基础上加用新鲜冰冻血浆,治疗后比较两组患儿的凝血酶原时间(PT)、部分凝血活酶时间(APTT)、纤维蛋白原(FIB)、D-二聚体(D-D)和颅内出血发生率。结果治疗后两组PT、APTT、FIB、D-D4个指标均有好转,实验组各指标数值改善更加明显。两组比较发现PT(t=-2.65,P<0.05)、APTT(t=-2.49,P<0.05)、FIB(t=3.89,P<0.01)3项差别有统计学意义,而D-D项比较差别无统计学意义(t=-1.79,P>0.05)。查CT后发现实验组颅内出血发生率为12%,而对照组为34%,二者比较差别有统计学意义(χ2=4.28,P<0.05)。结论早产极低出生体质量儿在常规治疗的基础上加用新鲜冰冻血浆可较显著地改善其凝血功能,对颅内出血起到一定的预防作用。Objectives To explore the preventive effect of fresh frozen plasma in preveming intracranial hemorrhage in Preterm VLBWI. Method Preterm VLBWI born in the TungWah hospital affiliated to Sun Yat-sen university were chosed as the objects of the study. They were randomly divided into experiment group containing 29 cases and control group containing 32 cases. All cases adopted the conventional treatments to prevent the occurrence of intracranial hemorrhage. The patients in experimental group were administrated with fresh frozen plasma extra. PT, APTT, FIB, D-D and the incidence of intracranial hemorrhage were analyzed after intervention between two groups. Results The parameter of PT,APTT,FIB and D-D of both groups were improved. The parameter of PT( t = - 2.65, P 〈 0.05 ), APTY( t = - 2.49,P 〈 0.05 ), FIB ( t = 3.89,P 〈 0.01 ) of the experimental group were better than that of the control group statistically. There has no difference between two groups at the level of D- D( t = - 1.79 ,P 〉0.05 ). The incidence of intracranial hemorrhage between two groups has statistical significance ,the rate of experimental group is 12%, while the rate of control group is 34%. (X^2 = 4.28,P 〈 0.05 ). Conclusion The preventative application of fresh frozen plasma in preterm VLBWI can improve the function of coagulation and has the effect on preventing intracranial hemorrhage.
