摘 要:目的观察失血性休克及复苏对大鼠肺部中性粒细胞浸润和肺微血管通透性损伤的作用。方法雄性SD大鼠,体重300~350g,麻醉后分离股动脉和颈内静脉,股动脉置管测压,从颈静脉缓慢抽血或回输,将平均动脉压降至35~40mmHg,维持90min。假休克大鼠也进行麻醉和置管。然后失血性休克大鼠按每失血1ml给予3ml乳酸林格氏液复苏,假休克大鼠按同体重失血性大鼠失血量的2倍量乳酸林格氏液复苏,复苏时间30min。复苏后3h,存活大鼠重新麻醉,随后开胸取样行肺部髓过氧化物酶分析和肺微血管通透性。结果血压监测显示,失血性休克及复苏模型制作良好。与假休克组相比,失血性休克大鼠肺部PMN聚集增加、PMVP降低(P<0.01)。结论失血性休克及复苏后可引起大鼠模型肺损伤。Objective The aim of the present study was to determine the effects of hemorrhagic shock and resuscitation on neutrophils sequestration in lung and pulmonary microvascular permeability in rats. Methods Male Sprague-Dawley rats were anesthetized with pentobarbital sodium. The rats undergoing hemorrhagic shock had blood withdrawn through the jugular vein catheter until their mean arterial pressure reached 35 to 40 mmHg. Mean arterial pressure was maintained at this level for 90 min with continuous monitoring of blood pressure and withdrawal and return of blood as needed. Sham shock groups were subjected to the same anesthetic and cannulation procedures but were not subjected to hemorrhagic shock. At the end of the 90-min shock or sham shock period, hemorrhagic shock animals were resuscitated with 3 mL of lactated Ringer Solution per 1 mL of shed blood, while sham shocked rats received 2 mL of lactated Ringer solution per 1 mL of shed blood of their hemorrhagic companies with equivalent body weight. Three hours after resuscitation, surviving mice were re-anesthetized, and a tnoracotomy were performed to determine PMN sequestration in lung using MPO analysis, and PWVP using Evans blue permeability in lung. Results Continuous monitoring of blood pressure showed that the hemorrhagic shock and resuscitation model was successful. The pulmonary MPO levels and the PMVP levels showed statistically significant increase in hemorrhagic rats, when compared with those sham shocked controls (P〈0.01). Conclusions Hemorrhagic shocked and resuscitation can cause significant lung injury.
