摘 要:目的:观察低糖透析液对终末期糖尿病肾病(DN)患者血液透析中血糖(BS)及血浆胰岛素(INS)水平的影响。方法:30例DN患者,随机平均分成两组,分别采用低糖透析液和无糖透析液进行血液透析治疗,所有研究病例在血液透析前、血液透析开始后1h、2h、3h和血液透析结束时作BS、INS测定。结果:无糖组血液透析时部分患者出现低血糖等不良反应,而低糖组血液透析时所有患者较少出现低血糖等不良反应。两组患者血液透析过程中血糖呈下降趋势,无糖组较低糖组血糖下降明显(P<0.05);两组患者INS无显著变化,且各时间点无显著差异(P>0.05)。结论:血液透析治疗DN时使用低糖透析液能减少低血糖事件的发生,对血糖水平影响较少,能更好地控制血糖,优于无糖透析液。Objective:To investigate the low carbohydrate dialysate on BS and INS in the treatment of diabetic nephropathy in final stage. Methods:30 DN patients were radomly divided into two groups. One group received low carbohydrate dialysate and the other received non-carbohydrate dialysate. The level of BS and INS were detected before hemodialysis and after 1h ,2h, 3h and the end of hemodialysis. Results:Patients received non-carbohydrate dialysate developed hypoglycemia and other side effects, while patients re- ceived low carbohydrate seldom developed hypoglycemia or other side effects. Level of BS in both groups was decreasing according to the time of hemodialysis, while they decreased more significantly in non-carbohydrate group than that in low carbohydrate group (P〈0.05). Level of INS in both groups did not differ significantly according to the time of hemodialysis. Conclusions: Low carbohydrate dialysate can reduce the incidence of hypoglycemia in the hemodialysis for DN patients. It is better than non - carbohydrate in the controling of blood sugar.
