摘  要:目的研究老年高血压患者脉压与反映肾脏早期损害的尿微量蛋白之间的相关性。方法将213例老年原发性高血压患者根据脉压水平分为4组(〈50mmHg、50—59mmHg、60—69mmHg、≥70mmHg)。所有患者人选时血尿素氮、肌酐和24h尿蛋白总量均在正常范围内。取晨尿用免疫散射法测定尿白蛋白(A1b)、转铁蛋白(TRF)、α1-微球蛋白(α1-MG)和免疫球蛋白G(IgG)。结果脉压升高,各项尿微量蛋白指标均有增高趋势,但仅有尿A1b、TRF和IgG3项指标与脉压水平同步增高(P〈0.05)。脉压〈60mmHg组的尿IgG水平在正常范围内,不同脉压组的尿α1-MG水平均在正常范围内。结论脉压可作为肾动脉粥样硬化、肾功能损害的高危预测指标之一。测定尿微量蛋白可作为早期高血压肾脏损害的诊断和监测指标。Objective To study the relationship between pulse pressure (PP) and urine mieroprotein which is the reflector of early damage to the renal function in elders with essential hypertension. Methods 213 elderly hpertensive patients with normal serum urea nitrogen( BUN), ereatinine (Cr) and 24h urine protein were divided into four groups by the level of PP( 〈 50 mmHg,50 - 59 mmHg,60 - 69 mmHg,≥70 mmHg). Urine albumine ( A1b), transferrin(TRF), α1-mieroglobulin (α1-MG) and IgG were determined by immunonephelometrie system. SPSS 11.5 was used to analyse all the data by one way ANOVA. Results In the four groups, the levels of urine Alb, TRF and IgG increased synchronously with the PP levels (P 〈 0.05 ). The levels of urine IgG were in the normal range, when PP were 〈 60mmHg. The levels of urine α1-MG were in the normal range in all the four groups. Conclusions PP is one of the high risk prediction factors of renal arteriosclerosis and renal function damage. Determination of urine mieroprotein, which is a monitor mark, is useful for the diagnosis and monitor of renal damage in the earlier stage.
