摘  要:目的研究异氟烷预处理对缺血再灌注损伤大鼠心肌梗死面积的影响,探讨其对心肌缺血再灌注损伤的保护作用。方法建立大鼠心肌缺血再灌注损伤模型,所有大鼠都平衡30min、给予异氟烷30min(除对照外)、异氟烷排除15min、心肌缺血30min和再灌注2h。实验将48只雄性Sprague Dawley大鼠随机分为6组:CON组、DMSO组、PTN组、ISO组、PTN+ISO组和ISO+PTN组。CON组无任何预处理,单纯心肌缺血再灌注;DMSO组于平衡15min时腹腔注射二甲亚砜(DMSO);PTN组于平衡15min时腹腔注射NF-κB特异性抑制剂小白菊内脂(parthenolide,PTN)500μg/kg;ISO组吸入1.0MAC异氟烷(ISO)30min,15min药物排出;PTN+ISO组和ISO+PTN组分别于异氟烷预处理开始前、后15min腹腔注射PTN500μg/kg。实验中监测血液动力学变化,实验结束用氯化三苯四唑(TPT)染色法测定心肌梗死面积。结果平衡期时,各组间心率(HR),平均动脉压(MAP)和心率-收缩压乘积(RPP)的差别均无统计学意义(P>0.05)。ISO组、PTN+ISO组和ISO+PTN组在吸入异氟烷期间HR、MAP和RPP明显降低(P<0.05)。再灌注1h和2h引起各组MAP和RPP明显降低(P<0.05)。与CON组(52.48±6.04)%相比,ISO组的心肌梗死面积显著减少(36.82±4.99)%,而PTN+ISO组(51.56±3.15)%和ISO+PTN组(51.06±1.05)%的心肌梗死面积明显高于ISO组(36.82±4.99)%(P<0.05)。结论异氟烷预处理使大鼠心肌梗死面积减少,对心肌缺血再灌注损伤有一定的保护作用。PTN可阻断此作用,推测NF-κB可能介导了异氟烷预处理的心肌保护作用。Objective To investigate the myocardial infarct size and the protective effect of isoflurane preconditioning on myocardium against ischemia-reperfusion injury in rats and the possible mechanism. Methods Myocardial ischemia-reperfusion injury model was instrumented in Sprague Dawley (SD) rats. All rats, except for the control group, were subjected to 30 min of equilibration, 30 min of isoflurane exposure and 15 min washout, followed by 30 min of left coronary artery occlusion and 2 h of reperfusion. Forty eight male SD rats were randomly divided into six groups. CON group, DMSO group,PTN group,ISO group,PTN + ISO group and IS()+ PTN group. CON group: rats experienced myocardium ischemia-reperfusion merely; DMSO group: dissolvent DMSO was administered intraperitoneally (IP) 15min after equilibration ; PTN group : nuclear Factor-κB (NF-κB) inhibitor PTN (500 μg/kg) was administered IP as before ; ISO group : rats received isoflurane ( 1.0 MAC) for 30 min and 15 min wash-out followed by a 30 minutes occlusion;PTN + ISO group:PTN administered IP 15 min before exposure to isoflurane; ISO + PTN group: PTN administered IP 15 min after exposure to isoflurane. Hemodynamic changes were monitered during the experiment. Myocardial infarct size was determined using triphenyltetrazolium chloride staining. Results There was no statistically significant difference in HR, MAP and RPP among all groups during the baseline (P〉0.05). During 30 minutes of isoflurane administeration, there was statistically significant difference in HR, MAP and RPPamong ISO group, PTN+ ISO group and ISO+ PTN group (P〈0. 05). 1 h and 2 h after reperfusion, there was a significant difference in MAP and RPP among all groups (P〈0.05). Infarct size in ISO group was significantly (P〈0.05) reduced (36.82 ± 4.99) % compared with control group (52.48 ± 6.04) % (P〈0. 05).
