膝关节滑膜炎关节镜手术配合几丁糖注射疗效分析

摘　　要：目的探讨膝关节滑膜炎关节镜手术配合几丁糖注射的临床疗效。方法135例诊断明确经保守治疗无效的膝关节滑膜炎患者,随机分为两组:A组68例,关节镜手术后关节腔内注射几丁糖;B组67例,膝关节镜手术后腔内不行药物注射,观察治疗效果。结果106例获得随访,平均21个月,采用林志雄评分标准评价疗效,同组术后与术前比较均有明显改善(P<0.05);A组术后半年和1年的随访结果优于B组(P<0.05),但术后1个月时比较无明显差异(P>0.05)。结论膝关节镜手术配合关节腔内注射几丁糖疗效确切,创伤小,术后并发症少,是治疗膝关节滑膜炎的理想方法之一。Objective To explore the clinical therapeutic effect of arthroscopy and chitosan intra-articular injection for gonarthromeningitis.Methods 135 gonarthromeningitis cases were randomized into 2 groups.68 cases of groups A were performed chitosan intra-articular injection following arthroscopy,while 67 cases of group B were underwent arthroscopy merely.Results 106 cases were followed up for average 21 months.In both 2 groups,the curative effect scores of post-operation were less than that of pre-operation in intra-group comparison（P〈0.05）.The effect of group A was better than that of group B in 0.5-and 1-year after operation（P〈0.05）,but no obvious difference in 1 month after operation between two groups（P〉0.05）. Conclusion The therapy of knee arthroscopy and chitosan intra-articular injection is one of an ideal management for gonarthromeningitis with advantages as definite effect,mini invasion and less complication.
