肝素钙综合治疗婴幼儿哮喘的疗效及机制

摘　　要：目的:探讨肝素钙治疗婴幼儿哮喘的效果及可能机制.方法:哮喘患儿68例随机分为对照组和治疗组,前者常规抗炎、平喘和对症治疗,后者在此基础上加用肝素钙静脉滴注.治疗前、后进行血小板计数,并检测患儿活化部分凝血活酶时间(APTT),D-二聚体(D-dimer),血清抗体IgE,IgA1和IgA2.结果:治疗组和对照组的总有效率依次为88.9%和62.5%,差异有统计学意义(P<0.01).与治疗前相比,治疗后治疗组患儿PL平均下降288×109/L,APTT水平平均上升9.65 s,D-dimer水平平均下降0.31 mg/L,差异具有统计学意义(P<0.01);治疗后治疗组患儿血清IgE平均下降116.7 mg/L,IgA1升高1751.1 mg/L,IgA2升高54.9 mg/L,差异均有统计学意义(P<0.01).结论:肝素钙辅助治疗婴幼儿哮喘效果较好,其可缓解高凝状态,改善患儿免疫功能.AIM： To explore the efficacy of calciparine on asthmatic infants and its possible mechanism. METHODS： Sixtyeight asthmatic infants were divided into the control and treatment groups. The controls were treated with routine anti-inflammatory, antiasthmatic and symptomatic treatment, and on the basis of these basic treatment, calciparine was added in the treatment group. The platelet counting, activated partial thromboplastin time （APTT） , D-dimer, serum IgE, IgA1 and IgA2 were determined. RESULTS： The total efficacy rates of the treatment and control groups were 88.9% and 62.5% , respectively, with significant difference （ P 〈 0.01 ）. After treatment, the number of platelet was decreased by 288 ×10^9/L, the level of APTT was increased by 9.65 s, the level of D-dimer was decreased by 0.31 mg/L, and significant differences were observed between pre- and post- treat- ment （ P 〈0.01 ）. The level of IgE was decreased by 116.7 mg/L, the levels of IgAland IgA2 were increased by about 1751.1 mg/L and 54.9 mg/L after treatment, significantly different as compared with those before treatment （P 〈0.01）. CONCLUSION： Calciparine has good effects on asthmatic infants, which can relieve hypercoagulability and improve immune function in these infants.
