摘  要:目的探讨核因子κB(NF-κB)在单肺通气所致肺损伤中的作用及对炎性因子的影响。方法12只日本大耳白兔随机分为两组各6只。双肺通气组(TLV组):双肺通气3 h;单肺通气组(OLV组):采用自制双腔气管导管行右侧单肺通气2 h,随后恢复双肺通气1 h。开胸取肺组织,其中OLV组左右肺分开处理,记为OLVL和OLVR。采用凝胶电泳迁移率改变分析法(EMSA)检测各组肺组织核蛋白提取物中NF-κB活性,Western blot法测肺组织抑制蛋白(IκB-α)含量,RT-PCR检测肿瘤坏死因子α(TNF-α)和细胞间黏附分子-1(ICAM-1)mRNA的表达,同时测肺组织湿/干重比并做肺组织病理学检查。结果与TLV组比较,OLV组的OLVL和OLVR的湿/干重比明显增高(P<0.05或P<0.01),同时OLVL湿/干重比又明显高于OLVR(P<0.01)。OLVL和OLVR的NF-κB活性均增高(P<0.01);两侧肺比较,OLVL高于OLVR(P<0.01)。IκB-α的含量OLVL和OLVR与TLV组比较均明显降低(P<0.01),且OLVL低于OLVR(P<0.01)。OLVL和OLVR中TNF-α和ICAM-1 mRNA的表达均高于TLV组(P<0.01),且OLVL高于OLVR(P<0.05)。结论NF-κB可能在单肺通气所致肺损伤中起到了重要的调控作用。Objective To study the changes of nuclear factor-κB (NF-κB) activation and expression of cytokines in lung tis'sues after one lung ventilation (OLV) in the rabbit models. Methods Twelve white Japanese rabbits were randomly divided into 2 grottps with 6 animals each. Two-lung ventilation (TLV) was performed in gro,up TLV for 3 h. OLV was performed for 2 h followed,by TLV for 1 h in group OLV. Lung separation was achieved with an artificial double-lumen tube. The lung tissue samples of group OLV were marked as OLVLand OLVR. respectively. NF-κB activity in nuclear protein from lung tissues was measured by electrophoretic mobility shift assay (EMSA). quantity of IκB-α by Western blotting and expression of TNF-α and ICAM-1 mRNA by RT-PCR. Light microscopic evaluations were performed and W/D ratio was determined. Results NF-κB activity, quantity of IκB-α, the expressions of TNF-α and ICAM-1 mRNA were significantly different between group TLV and OLV (P〈0.01). To comppared with OLVR, NF-κB activity of OLVL was significantly increased while quantity of IκB-α was significantly decreased (P〈0.01). The expressions of TNF-α and ICAM-1 mRNA of OLVL were higher than those of OLVR (P〈0.05). Conclusion, IκB-α degradation and NF-κB activation probably play an important role in lung injury caused by OLV.
