咳喘胶囊治疗支气管哮喘慢性持续期15例临床观察

摘　　要：目的观察咳喘胶囊治疗支气管哮喘慢性持续期(风痰阻肺证)的临床疗效。方法将35例患者随机分为治疗组18例与对照组17例,研究过程中2组实际完成观察病例数均为15例。治疗组口服咳喘胶囊每次4粒,每日3次;对照组吸入布地奈德干粉剂每次200μg,每日2次,两组疗程均为28天;观察疾病疗效、主要症状疗效及肺功能一秒用力呼气容积(FEV1)、呼气高峰流量(PEFR)和最大呼气流量(PEF)昼夜变异率的变化。结果疾病疗效方面,治疗组临床愈显率为60.0%,对照组临床愈显率为66.7%,两组比较差异无统计学意义(P>0.05);主要症状疗效方面,两组治疗对哮鸣音改善的愈显率差异无统计学意义(P>0.05),对胸膈满闷症状的改善两组愈显率差异有统计学意义(P<0.05);对肺功能改善方面,两组自身治疗前后对FEV1和PEFR指标的改善差异均有统计学意义(P<0.05),但是两组治疗后FEV1和PEFR变化的比较差异无统计学意义(P>0.05);对PEF昼夜变异率影响方面,两组治疗前后自身比较和治疗后组间比较差异均有统计学意义(P<0.05)。结论咳喘胶囊治疗支气管哮喘慢性持续期(风痰阻肺证),能够显著缓解哮喘的症状和体征,明显改善肺通气功能,在解除胸膈满闷症状及降低PEF昼夜变异率方面有一定的优势。Objective To observe the effect of Kechuan Capsule on the syndrome of wind-phlegm retention in lung at the persistent stage of bronchial asthma. Methods Totally 35 cases were randomly divided into treatment group （18 cases） and control group （17 cases）, and 15 cases of each group completed the study. Patients in the treatment group were administrated Kechuan Capsule, 4 capsules each time, 3 times a day; while those in the control group inhaled budesonide 200μg each time, twice a day. The course of treatment was 28 days. The effect on asthma episode, main symptoms, forced expiratory volume in 1 second （FEV1）, peak expiratory flow rate （PEFR） and variation rate of PEF in 24 hours were observed. Results The effective rate of the treatment group was 60.0% while that of control group 66.7%, without significant difference between two groups （P〉0.05）. In relieving wheeze, the effective rate of both groups was the same, 73.3 %. In reliving chest distress, the effective rate of treatment group was 73.3% and that of control group 33.3 %, with a significant difference （P〈0.05）. FEV1 and PEFR of lung function of both groups were improved significantly after treatment （P〈0. 05）, but no significant difference was found between groups （P〉0. 05）. The variation rate of PEF of two groups in 24 hours were significantly different in in-group and between-group comparisons （P〈0.05）. Conclusion Kechuan Capsule can relieve the symptoms of wind-phlegm retention in lung of bronchial asthma at persistent stage, improve the lung function in respiration and inhibit the episode of asthma. The treatment group was superior to the control group in relieving chest distress and decreasing variation rate of PEF in 24 hours.
