脑梗死后抑郁症患者百忧解治疗前后认知功能和情感障碍以及事件相关电位的变化

摘　　要：目的研究和探讨脑梗死后认知障碍和情感障碍的状况以及抗抑郁治疗对认知功能和认知事件相关电位P300波幅和潜伏期的影响。方法收集神经内科住院的脑梗死后抑郁症患者112例,正常对照组56例。分别进行24项HAMD抑郁量表和韦氏成人智力量表测量,同时进行事件相关电位P300波幅和潜伏期的检查,然后进行统计分析。结果抗抑郁剂治疗后脑梗死抑郁症患者的认知功能明显改善,抑郁量表评分降低。且患者抑郁评分和认知功能评分呈明显负相关,伴随着治疗后HAMD评分的降低,患者P300的潜伏期缩短,波幅降低。结论及时早期抗抑郁治疗可明显改善患者的认知功能和情感障碍,改善认知事件相关电位P300的潜伏期和波幅,促进神经功能康复。Objective To study the cognitive function, P3 amplitude and P3 latency changes on the patients with post-stroke depression after treatment. Methods Collecting 112 cases with the post-stroke depression that were given in fluoxetine for 12 week in hospital of our department of neurological and the normal contrast groups of 56 cases, We evaluated the cognitive function and the emotional status separately with Wechsler adult intelligence scale （WAIS- RC） and 24 -item Hamilton Depression Scale （HAMD） , and detected the event-related potential P300, then carried on statistical analysis. Results The HAMD scores of patients with post-stroke depression were decreased significantly （P〈0.01） and the cognitive function scores of them were increased significantly in post treatment. Total HAMD score and cognitive function scores of them were negatively correlated significantly after treatment. Their latency period of P3 was shortened and the amplitude of P3 was prolonged （P 〈0.01）. However, their cognitive function scores were still lower than the normal contrast groups, their latency period of P3 was longer and the amplitude of P3 was lower than the normal contrast groups. Conclusion The cognitive function with post-stroke depression patients has significantly improved after treatment. The improvement of cognitive function was correlated to the remission of the emotional disorder. Although these patients＇ emotional disorder syndrome was remission, the cognitive function and the amplitude of P3 and the latency period of P3 of them could not recovered to normal level.
