先天性上睑下垂矫正术后早期眼表变化的研究

摘　　要：目的探讨先天性上睑下垂矫正术后眼表的早期改变。方法对先天性上睑下垂患者37例(42眼)分别于矫正术术前及术后1d、7d、1个月和3个月行泪膜破裂时间、泪液分泌试验、角膜荧光素染色、下睑泪河高度测量和比较,询问和记录患者不适感。结果患者术后1d和7d角膜荧光素染色点明显增多,分值明显高于术前,差异有统计学意义(P1=0.007,P2=0.006)。而术后泪膜破裂时间、泪液分泌量与术前比变化不明显,差异均无统计学意义(P>0.05)。提上睑肌缩短术及额肌肌瓣悬吊术后眼表的各项指标比较差异也无统计学意义(P>0.05)。结论先天性上睑下垂矫正术后早期角膜上皮受到损害。在使用人工泪液及眼膏的情况下,泪膜破裂时间及泪液分泌量无明显变化,手术方式对眼表的改变无明显差异。Objective To investigate the early changes of tear film in patients with congenital ptosis after surgical correction. Methods The tear film break-up time, SchirmerItest, fluorescein examination before and after levator muscle excision and frontalis muscle flap suspension were examined at 37 patients （42 eyes ） with congenital ptosis. Their discomforts were asked and recorded. Results There was signifi- cant difference in the corneal epithelial staining spots before and after surgery（ P1=0. 007 ,P2 =0. 006）. The corncal epithelial staining spots increased within one week postoperatively. There was no significant difference statistically in the tear film breakup time and SchirmerItest when patients used the artificial tears and eye ointment. The changes of the ocular surfaces had no statistical difference between the patients with eyelid levator excision or frontalis flap suspension（ P 〉 0.05）. Conclusion The corneal epithelium is slightly damaged in the early period after statical correction for congenital ptosis, but tear film breakup time and SchirmerItest do not change. The surgical methods for correcting congenital ptosis do not affect the changes of ocular surfaces. [Rec Adv Ophthalmol 2008;28（8） ：607-609]
