摘 要:目的应用组织多普勒应变率成像(SR I)技术评价不同舒张功能的高血压病患者左心房功能变化。方法选择原发性高血压病患者54例,依照超声心动图测定的舒张功能分为A组(舒张功能正常者)31例和B组(舒张功能轻度减低者)23例;对照组为12例健康志愿者。应用SR I技术获取所有入选者左心房后壁的组织应变及应变率曲线,进一步获得舒张早期应变率E峰峰值(ESR)、心房收缩期应变率A峰峰值(ASR)及左心房应变峰值(ASS),观察以上参数在各组间的差异及与左心房射血力指数(LASFi)、左心房主动排空分数(LAAEF)及左心房每搏量指数(LASV i)的相关性。结果ASS在正常组与A组间差异无统计学意义(P>0.05),但B组高于正常组和A组(P<0.01);左心房应变率指标ESR、ASR与应变相似,在正常组与A组间差异无统计学意义(P>0.05),但B组高于正常组和A组(P<0.01)。ASR与LASFi呈显著负相关(r=-0.525,P<0.01),ASS与LAAEF、LASV i呈显著负相关(r=-0.509、r=-0.650,P均<0.01)。结论SRI技术可用于评价高血压病患者左心房功能变化。Objective To evaluate the changes of the left atrial function in the patients with hypertension classified by diastolic states in virtue of the strain rate analyzing. Methods The study population consisted of 54 patients with hypertension and 12 healthy volunteers (control group). The patients with hypertension were further grouped into 2 cohorts according to the echocardiographic diastolic function. Group A included 31 patients with normal diastolic function; Group B included 23 patients with mildly impaired diastolic function. The longitudinal strain rate (SR) and strain (ε) were measured in the posterior LA wall and the relationship between the SR/ε profdes and the left atrial functional parameters including the left atrial systolic force index (LASFi), left atrial stroke volume index ( LASVi ) and left atrial active emptying fraction (IAAEF) were analyzed in the groups. Results No significant difference in ASS was observed in the group A and control group. The ASS in group B was higher than those in group A and control group as well (P 〈 0. 01 ) ; The ESR、 ASR and ASS were similar between group A and control group. Both ESR and ASR in group B were higher than group A and contraol grouep (P 〈0. 01 ). The ASR was postively correlated with LASFi (r = -0. 525 ,P 〈0. 01 ) and negatively correlated with LASVi and IAAEF(r = -0. 509 ,P 〈 0. 01 and r = -0. 650,P 〈 0. 01 ). Conclusion SR/ε imaging could be used as a new method to assess left atrium function, which might be helpful for awareness of the physiological and pathophysiological changes of left atrial function in the patients with hypertension.
