经尺骨鹰嘴入路关节外斜行截骨对血供影响解剖学观察

摘　　要：目的:为尺骨鹰嘴关节外斜行截骨血供与骨折愈合提供解剖依据。方法:新鲜成人上肢标本20侧,以红色乳胶血管灌注,解剖观测了肱三头肌腱的营养血管分布情况;尺骨近段血管骨膜支的分布,走形和外径。结果:肱三头肌腱表面细小动脉血管呈网分布,肱三头肌腱深面滑膜处恒定有2￣3支外径为0.74￣1.02mm的肌穿支。尺骨近鹰嘴段,内侧有1￣2条较其它肌骨膜支粗大血管,外径0.62￣0.94mm;外侧恒定有一骨膜支,在出旋后肌水平血管外径(1.58±0.16)mm,平桡骨颈水平外径(1.17±0.10)mm。结论:经尺骨鹰嘴关节外斜行截骨处,血管分支丰富,肌肉附着点多,稳定固定后截骨片易于愈合。Objective： To provide anatomic basis for the effects of the approach of extra-articular oblique osteotomy of olecranon on blood supply and-the healing of the fracture. Methods： 20 adult fresh upper limbs perfused with red latex were dissected. The distribution and the diameter of blood vessels reached to triceps tendon and the periosteum surrounding the proximal part of the ulna were observed and measured. Results： The triceps tendon was covered by deep fascia which contained dense arterial vascular net. 2-3 muscular perforating branches existed constantly in the deep synovium of the olecranon with the external diameter of （0.74-1.02） mm. 1 to 2 branches with the external diameter of （0.62-0.94）mm can be found at the medial side of the proximal end of the ulna, as well a branch of periosteum at the lateral side with the diameter of（ 1.58±0.16）mm at the level of the supinator, and （ 1.17±0.10）mm at the level of the neck of radius. Conclusions： Plentiful blood supply and the numerous muscular attachment point ensure the better fracture healing after extra-articular oblique osteotomy of olecranon.
