益气活血中药对肺癌放疗患者肺功能的影响

摘　　要：目的观察益气活血中药对非小细胞肺癌患者放疗前后肺功能的影响,探讨其在放射性肺损伤防治中的作用。方法126例未手术的Ⅲ期非小细胞肺癌患者,治疗组71例,对照组55例,卡氏评分≥80分,均行三维适形放疗,治疗组加服益气活血中药,放疗前及放疗后1、3个月检查肺功能主要参数用力肺活量(FVC)、1秒用力呼气量(FEV1)、肺一氧化碳弥散量(CLCO),同时观察放射性肺炎的发生率及程度。结果所有患者均完成放疗,剂量为66～70Gy,治疗组放疗后3个月的肺功能与放疗后1个月比较无统计学差异(P>0.05),对照组放疗后3个月肺功能明显低于放疗后1个月(P<0.05);治疗组与对照组放射性肺炎的发生率分别为15.49%、30.90%,相比具有统计学差异(P<0.05),其中Ⅱ+Ⅲ级分别为2.82%、12.72%,相比具有统计学差异(P<0.05)。结论益气活血中药可明显改善肺癌放疗患者的肺功能,降低放射性肺炎的发生率。Objective To investigate the effect of Yiqi Huoxue Recipe on the pulmonary function of non-small-cell lung cancer patients after radiation therapy and explore its effect on lung injury of radiotherapy. Methods One hundred and twenty six patients of III phase lung cancer （therapy group 71, control group 55, KPS≥80） received three-dimensional conformal radiation therapy, and the therapy group received Yiqi Huoxue Recipe. The pulmonary function indexes of FVC, FEV1 and CLCO was tested before radiotherapy and in M1 and M3 after radiotherapy, and the appearance and grade of radiotherapy pneumonitis was observed. Results All patients completed radiotherapy, and total dose was 66-70 Gy. The pulmonary function in M3 after radiotherapy was similar to that in M1 after radiotherapy of therapy group （P〉0.05）. The pulmonary function in M3 after radiotherapy were lower than that in M1 after radiotherapy of control group （P〈0.05）. The incidence rate of two groups were 15.49% and 30.90% respectively （P〈0.05）. The grade of Ⅱ and Ⅲ were 2.82% and 12.72% respectively （P〈0.05）. Conclusion Yiqi Huoxue Recipe can enhance the pulmonary function and degrade the incidence rate of radiotherapy pneumonitis of Ⅲ phase non-small-cell lung cancer patients.
