通腑平喘汤对COPD机械通气患者呼吸力学的作用

摘　　要：目的:观察通腑平喘汤对慢性阻塞性肺疾病(COPD)机械通气患者呼吸力学的影响。方法:将符合诊断标准的COPD并呼吸衰竭患者随机分为治疗和对照两组,各23例。两组均采用机械通气等西医常规治疗,治疗组加用通腑平喘汤,1天1剂,煎汁200ml,分2次鼻饲。比较两组治疗前、治疗后3、7天的氧合指数(PaO2/FiO2)、肺顺应性(Cdyn和Cst)、气道峰压(PIP)、气道平台压(Pplat)及气道阻力(Raw)。结果:治疗组治疗7天时PaO2/FiO2、Cydn、PIP、Pplat及Raw均较对照组显著改善(P均<0.05),尤以Cst改善明显(P<0.01)。结论:通腑平喘汤有助于尽快改善COPD呼吸衰竭患者的呼吸功能。Objective：To investigate the effect of Tongfu Pingchuan decoction on respiratory mechanics of patients with chronic obstructive pulmonary disease （COPD） treated by mechanical ventilation （MV）. Methods： COPD patients were randomly divided into the treatment group and the contrast group with 23 cases each group. Routine western medicine including MV was used in two groups. The treatment group was added with one dosage of 200 ml Tongfu Pingchuan decoction infused via nasal tube twice a day. Indexes such as PaO2/FiO2, Cdyn, Cst, PiP, Pplat and Raw before therapy, on the 3^rd and 7^th day after therapy were compared. Results： Indexes including PaO2/FiO2, Cdyn, PiP, Pplat and Raw of the treatment group on the 7^th day were obviously improved than those of the contrast group （P 〈 0.05）. Among them, the improvement of Cst was most （P 〈0.01 ）. Conelusion：Tongfu Pingchuan decoction can improve the respiratory function of COPD rapidly.
