摘 要:目的观察肾移植术中应用小剂量多巴胺对移植肾功能的影响。方法肾移植术患者40例,随机分为多巴胺组和对照组各20例。术中分别以多巴胺和0.9%氯化钠微泵持续静推,速度为2μg·kg-1.min-1。观察出尿时间、移植肾血管开放后1h总尿量、移植肾血管开放后6h总尿量;手术开始(T1)、移植肾动脉开放前10min(T2)、移植肾动脉开放后10min(T3)、移植肾动脉开放后1h(T4)、移植肾动脉开放后6h(T5)等5个时间点的静脉血中肌酐、尿素氮结果。结果多巴胺组出尿时间显著快于对照组(P<0.05);多巴胺组T4、T5的总尿量显著高于对照组(P<0.05)。多巴胺组T5静脉血中肌酐、尿素氮显著低于手术开始时(P<0.01)。多巴胺组T5的静脉血中肌酐、尿素氮两项结果明显低于对照组(P<0.05)。结论术中使用多巴胺以2μg·kg-1.min-1的速度微泵持续静推,能明显促进早期移植肾功能的恢复。Objective To investigate the effect of low-dose dopamine on allograft function in kidney transplantation operation. Methods Forty kidney transplantation patients were divided into two groups,the dopamine group and matched control group randomly,20 cases each. Dopamine (2μg·kg^-1·min^-1) and 0.9% NaCl (2μg·kg^-1·min^-1 ) were given by micro-pump. Observed the urine time,total urine quantity of one hour and 6 hours after the transplanted kidney vessel openned, the time of surgical operation beginning (T1), 10 mins before the transplanted kidney vessel openned (T2 ), 10 mins after the transplanted kidney vessel openned (T3 ), one hour after the transplanted kidney vessel openned ( T4 ) ,6 hours after the transplanted kidney vessel openned ( T5 ), the results of creatinine and urea nitrogen of vein bloods in those 5 time points. Results The urine time of the dopamine group was obviously quicker than that in matched control group ( P 〈 0.05). The total urine of dopamine group after the kidney vessel openned one hour and 6 hours all shows signoficantly higher than those in matched control group ( P 〈 0.05 ). Two results of the creatinine and the urea nitrogen of dopamine group of 6 hours after transplanted kidney vessel openned signoficantly lower than those of surgical operation beginning ( P 〈0.01 ). Two results of the creatinine and the urea nitrogen of dopamine group in 6 hours after transplanted kidney vessel openned signoficantly lower than those in matched control group ( P 〈 0.05 ). Conclusion The usage of dopamine ( micro-pump with the speed of 2 μg·kg^-1·min^-1 ) intravenous injection allograft in the kidney transplantation operation persistent, can obviously promote the instauration of early allograft function in kidney transplantation.
