摘  要:目的评价对危重患者大便失禁进行恰当的预防性治疗与护理的临床意义。方法选择2004年10月~2008年3月收治的共55例大便失禁的危重患者,随机分为实验组35例,对照组20例,实验组采用一次性气管插管(柔软气囊鼻插管)进行大便引流。对照组采用传统护理方法。对两种不同护理方法进行效果比较。结果两组患者肛周皮肤改变情况分析比较,实验组Ⅰ度2例(2/35,5.71%),对照组Ⅰ度6例、Ⅱ度3例、Ⅲ度1例(10/20,50.00%),实验组并发症发生率明显低于对照组(P<0.05)。结论对大便失禁的危重患者应用一次性气管插管引流能有效控制难治性湿症、压疮的发生,可取得良好的社会效益和经济效益。Objecti ve To evaluate the effect of disposable balloon catheter application for feces incontinence in critical patients. Methods Fifty-five critical patients with cerebral vascular accident, severe pancreatitis, cerebral trauma, or multiple organ failure, 35 males and 20 females, aged (65 ± 5) (33-87), who suffered from feces incontinence because of antibiotic-associated pseudomembranous enteritis or stress gastrointestinal hemorrhage caused by high-dose adrenal cortex hormone treatment were randomly divided into 2 sex-, age-, and state of illness-matched groups: experiment group (n=35) undergoing insertion of a disposable indwelling balloon catheter to the juncture of sigmoid colon and rectum and deflation every 4- 6 hours, and control group (n=20) receiving conventional nursing care. Observation was carried out for 7 days. Results The complication rate of the experiment group was 5.71%, significantly lower than that of the control group (50.00%, P〈0.05). Conclusion Application disposable balloon catheter for feces incontinence in critical patients effectively reduces the incidence of pressure sores and refractory eczema in critical patients.
