Treatment of Complex Renal Stone Under Percutaneous Nephrolithotomy with Pneumatic Lithotripsy and Holmium:YAG Laser Lithotripsy

摘　　要：目的探讨经皮肾镜下(PCNL)钬激光联合气压弹道碎石(PL+LL)治疗复杂性肾结石的疗效。方法回顾性分析我院276例经皮肾镜下PL+LL治疗的复杂性肾结石患者的临床资料。结果所有病例平均手术时间96min,术中出血量50～600ml,术后血尿时间1～7d,2例分别于术后第3、5d行选择性肾动脉栓塞止血。PL+LL一期结石清除率为70.3%(194/276),二期结石清除率87.1%(61/70),ESWL处理残留结石4例,排尽3例。结论经皮肾镜钬激光联合气压弹道碎石治疗复杂性肾结石安全、有效。Objective To evaluate the clinical effect of percutaneous nephro- lithotomy with pneumatic Iithotripsy and holmium： YAG laser lithotripsy for complex renal stone. Method The clinical data of 276 cases of complex renal stone treated under percutaneous nephrolithotomy （PCNL） with pneumatic Iithotripsy and holmium： YAG laser lithotripsy were analyzed retrospectively. Results Average operation time was 96 min, blood loss volume during the operation ranged from 50 to 600 ml. Time of blood urine was one to seven days. Two patients accepted selectivity thrombosis of renal artery in the 3rd and 5th days after operation respectively. The first stage stone- free rate was 70.3 % （ 194/276）, and the second stage stone - free rate was 87.1% （61/70）. Four patients accepted ESWL for residual stone, and three were free of stone. Conclusions Percutaneous nephrolithotomy with pneumatic Iithotripsy and holmium： YAG laser lithotripsy for complex renal stone is a safe and effective technique.
