低温下肾动脉阻断在肾脏手术中的临床应用(附41例报道)

摘　　要：目的：探讨局部低温肾动脉阻断在肾手术中的临床应用。方法：对32例复杂性肾结石患者在局部低温肾动脉阻断后行肾实质切开取石术．对6例肾错构瘤患者行肿瘤切除术，对2例肾结石分别合并左肾上、下盏重度积水患者和1例右侧重复肾患者行肾部分切除术。结果：阻断肾动脉时间15～50min，术中出血量120～400ml．手术时间120～180min，结石最大5．5cm×4．5cm×3．0cm，最多一次取石30余枚，肿瘤最大12．0cm×11．0cm×9．5cm，25例患者获随访，结石复发3例，1例术前IVU患肾显影浅淡患者，术后6个月复查IVU患肾不显影，其余患肾显影良好。结论：此术式具有方法简单、出血少、手术安全并能较好地保护患肾功能等特点，是较理想的手术方法之一，易于基层医院开展。Objective：To investigate the application of local renal artery interruption at low temperature for surgical removal of stones or tumors. Methods： The data included in this retrospective study were 32 cases with complex renal stones undergone nephrolithotomy, 6 with renal harnartoma undergone tumor resection, 2 complicated with upper and lower severe hydrccalycosis in left kidney received partial nephrectomy and 1 with renal duplication of resection of the right kidney by local renal artery interruption surgery at low temperature. Results ： Renal artery in- terruption consumed 15-50 min and blcvod loss was 120-400 ml with operation lasting for 120-180 min. The maximum sized stone was 5.5 cm× 4.5 cm×3.0 crn and more than 30 pieces were removed. The tumor removed was seen in a maximum sized with 12.0 cm× 11.0 cm× 9.5 crn. Follow-up of 25 patients revealed that they got excellent recovery except for 3 cases with relapse. Another 1 with preoperative iVU retardation returned normal. after 6 months of operation. Conclusion ： This approach is one of the preferable therapies with simple performance, less bleeding an.d safety, and it is recommended in local hospitals.
