摘  要:目的探讨内毒素血症时幼年大鼠急性胃黏膜损伤、胃黏膜上皮细胞、壁细胞超微结构变化及血小板活化因子(PAF)受体拮抗剂对其影响。方法将18日龄Wistar大鼠随机分为对照组、内毒素(LPS)组、RAF受体拮抗剂预防组和治疗组,对照组和LPS组分别腹腔注射生理盐水1ml/kg和LPS(O55:B5脂多糖)5mg/kg,PAF受体拮抗剂预防组和治疗组分别于LPS注射前、后0.5h给予注射PAF受体拮抗剂BN52021(GinkgolideB)5mg/kg。分别于注射后1.5、3、6、24、48、72h观察胃黏膜损伤情况,按Guth标准积分计算溃疡指数(UI);HE染色,光镜观察胃黏膜细胞形态学改变;钠、铅双染色透射电镜下观察胃黏膜上皮细胞、壁细胞超微结构变化;计算胃组织湿干比,即(湿重-干重)/湿重。结果LPS组LPS注射后6h胃黏膜损伤最重,黏膜表面可见大片糜烂、出血、条索状坏死,与胃纵轴平行;UI记分最高,与对照组、预防组、治疗组比较差异均有非常显著性(P〈0.01);光镜下黏膜表面上皮广泛脱落,黏膜内有出血,炎性细胞浸润,腺体受损;电镜下表面上皮细胞细胞间隙增宽,腔膜面破坏,细胞间紧密连接不连续,短小的微绒毛减少,线粒体严重肿胀,嵴断裂甚至消失,呈空泡变性,核固缩、核膜间隙增宽;壁细胞肿胀,盐酸小管扩张,部分有破坏呈空泡样改变,线粒体增多,部分空泡样改变。PAF受体拮抗剂预防组及治疗组改变轻微。结论内毒素血症时幼年大鼠急性胃黏膜损伤,胃黏膜上皮细胞、壁细胞超微结构发生变化;PAF受体拮抗剂可明显减轻其改变。PAF可能是引起内毒素血症时急性胃黏膜损伤的重要因素。Objective To investigate the changes of ultramicrostructure in gastric mucosal epithelial cell, parietal cell and effect of platelet activating factor(PAF) antagonist in young rars with endotoxemia. Methods Eighteenday old Wistar mrs were randomly divided into four groups:control, model, prevention and treatment group. The rats of control group and modal group were done with intraperitoneal injection of normal saline ( 1 ml/kg) and endotoxin (5 mg/kg of O55 :B5 lipopolysaccharide) respectively. The rats of prevention and treatment group were repectively administered with PAF antagonist BN52021 (GinkgolideB 5 mg/kg) half an hour before and after injected with endotoxin.And then, we observed the changes of gastric mucosa at 1.5,3,6,24, 48 and 72 hours after intraperitoneal injection of endotoxin, recorded the ulcer index(UI) of gastric mucosa according to Guth standard integral. The changes of celluar morphology and ultramierostructure of gastric muoosa were respectively observed by hematoxylin-eosin staining and transmission electron microscope. The ratio of wet and dry weight of gastric muoosa was also calculated. Resalts Gastric mucosal injury of rats were most serious at 6 hours after injection of endotoxin in model group, there was quite a lot of erosio, bleeding and necrosis and the UI score was highest in model group, there was highly significant difference between model group and other three groups (P〈0.01).There was shedding of surface epithelium, bleeding in muoosa, inflammatory cell infiltrate, and damage of glandular organ in model group, and also we could see that the ceil spaces of epithelial cells were widened, surface of cavity and muoosa was damaged, tight junction between cells Were damaged, microvilli decreased, and mitochondria were seriously sweUed. Also there was vacuolar degeneration and pyknosis, nuclear membrane spaces were widened. All the above changs were significantly alleviated in prevention and treatment group. Conclusion The gastric mucosa of young rats
