血清白蛋白浓度联合APACHEⅢ评分评估危重病人的预后

摘　　要：【目的】探讨ICU危重病人入院24h内急性生理学及慢性健康状况评估Ⅲ（APACHEⅢ）评分和血清白蛋白（SAB）水平改变与病情及预后关系。【方法】对90例入住ICU24h的病人进行APACHEⅢ评分；同时测定SAB浓度，并按SAB分为〈35g／L组、35～50g／L组和〉50g／L组，分析SAB和APACHEⅢ评分与病情及预后关系。【结果】〈35g／L组和〉50g／L组APACHEⅢ评分均高于60分，死亡率人数较35～50g／L组明显增多且差异有显著性。APACHEⅢ分值高低与病死率呈现正相关。【结论】APACHEⅢ评分和sAB浓度的改变对危重病人病情及预后有重要的相关性。[Objective] To explore the relationship of acute physiology and chronic health evaluation Ⅲ （A- PACHEⅢ ） and the changes of serum albumin（SAB）concentration with the state of illness and prognosis of patients admitted to intensive care unit （ICU） in 24 hours. [Methods] The evaluation variable of APACHE Ill was assessed, and the SAB concentrations were determined in 90 patients. All patients were divided into 35g/L group, 35-50g/L group and 〉50g/L group according to SAB. The relationships of SAB concentration and APACHE Ⅲ with state of illness and prognosis were analyzed. [Results] The APACHEⅢ in 〈35g/L group and 〉 50g/L group was more than 60, and the death rate of these two groups was significantly increased, compared with the 35-50g/L group. [Conclusion] There is positive correlation between APACHEⅢ and mortality. There is an important correlation of APACHEIII and the changes of SAB concentration with the state of illness and prognosis of patients.
