黄连素对慢性心力衰竭病人心功能及炎症因子的影响

摘　　要：目的观察黄连素对慢性心力衰竭病人心功能及炎症因子的影响。方法入选65例心力衰竭病人,根据纽约心脏病学会(NYHA)分级标准进行心功能分级,分别用ELISA法检测N末端脑钠素原(NT-ProBNP)、高敏C反应蛋白(hs-CRP)、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)浓度,超声心动图测左室射血分数(LVEF),并与正常对照组(30名)进行比较。心力衰竭病人随机分为治疗组和对照组,治疗2个月后复查。结果炎症因子hs-CRP、IL-6、TNF-α在心力衰竭心功能Ⅲ级、Ⅳ级组明显高于正常对照组(P<0.05);LN(NT-ProBNP)在正常对照组、心力衰竭组各心功能分级间两两比较差异均有统计学意义(P<0.05)。治疗后治疗组NYHA心功能分级明显改善,治疗组较对照组LVEF明显增加,LN(NT-proBNP)及hs-CRP、IL-6、TNF-α明显降低(P<0.05)。结论炎症因子hs-CRP、IL-6、TNF-α及LN(NT-ProBNP)与心力衰竭严重程度密切相关。黄连素改善心功能与炎症介质降低有关。Objective To study the effect of berberine on cardiac function and plasma factors in chronic heart failure （CHF）. Methods Sixty- five patients with CHF were randomized into conventional therapy and berberine therapy and followed up for 2 months. Plasma NT - ProBNP and proinflammatory cytokines high - sensitivity C - reactive protein （hs - CRP）, interleukin 6 （IL - 6） and tumor necrosis factor alpha （TNF - α） were determined by ELISA. Left ventricular ejection fraction （LVEF） was measured by ultrasonography. The results were compared with that in 30 healthy individuals. Results There was a significant difference in plasma hs - CRP , IL - 6 and TNF - α between healthy individuals and pts with class Ⅲ or class Ⅳ of cardiac function （ P 〈 0. 0 5 ） . Plasma LN （NT - ProBNP） was elevated with increasing of NYHA classification. Following treatment with berberine, cardiac .function was improved significantly in NYHA classification, LVEF and decrease significantly in LN, hs - CRP, IL- 6, and TNF - α （P〈0.05）. Conclusion Plasma LN, hs- CRP, IL- 6, and TNF-α were related significantly with CHF. Berberine was effective for CHF.
