摘  要:目的研究代偿性肾功能不全对心功能不全患者B型尿钠肽(BNP)浓度的影响。方法选择190例有心肌梗死病史的患者,测定血浆BNP、肌酐(Cr)浓度,超声心动图检查测定左心室射血分数(LVEF),记录性别、身高、体重,计算肾小球滤过率(GFR)。结果肾功能不全发生率为29%。肾功能不全患者BNP为(130±19)pg/ml,显著高于肾功能正常者的(65±6)pg/ml(P<0.05)。单因素及多因素变量分析BNP与GFR均相关(P<0.05)。结论BNP浓度受到轻中度肾功能不全的影响。Objective To assess the effect of compensated renal dysfunction on brain natriuretic peptide (BNP) in the patients after myocardial infarction. Methods BNP and creatinie(Cr) were measured in 190 patients with history of myocardial infarction, in whom left ventricular ejection fraction(LVEF) was measured by echocardiography. The glomerular filtration rate (GFR) was calculated. Results The morbidity of renal dysfunction (GFR〈85 ml/min) was 29% in this group. BNP in the patients with renal dysfunction was (130±19) pg/ml, which was significantly higher than (65±6) pg/ml in the patients without renal dysfunction (P〈0.05). BNP was correlated with GFR (P〈0.05). Conclusion BNP in the patients with history of myocardial infarction is influenced by mild to moderate renal dysfunction.
