三种方案治疗小儿幽门螺杆菌相关胃病中焦积热证前后C^(13)-呼吸试验DOB值变化

摘　　要：目的:观察三种方案治疗小儿幽门螺旋杆菌(H.Pylori)相关胃病中焦积热证前后的C13-呼吸试验DOB值变化,寻找最佳治疗方案。方法:108例H.Pylori胃病患儿按1∶1∶1对照原则随机分为3组:中药组、西药组和中西药组,每组各36例。中药组口服安幽颗粒(大黄、槟榔、黄连、丹参、香附、白芍);西药组口服克拉霉素、洛赛克;中西药组口服同疗程安幽颗粒加克拉霉素、洛赛克。结果:H.Pylor临床转阴率,中药组、西药组和中西药组分别为38.24%、15.63%、66.67%,中药组与中西药组相比,存在统计学差异;西药组与中药组、中西药组相比,存在显著统计学意义(P<0.05);呼气试验DOB值指标变化方面,西药组与中西药组间存在显著的统计学差异(P<0.01);西药组与中药组间存在显著的统计学差异(P<0.05);中药组与中西药组间存在统计学差异(P<0.05)。结论:中西药结合治疗能使H.Pylori相关胃病中焦积热证小儿C13-呼吸试验DOB值明显下降,并能提高H.Pylori临床转阴率。Objective： To observe changes of DOB value in C13 - breath test with three method treating children ’s H. pylori gastropathy of accumulation symptom of heat in middle -jiao and try finding the best thempy method. Method： Adopting a ratio of 1： 1：1 ,totally 108 H. pylori gastropathy chilren were randomed into three groups;the Traditional Chinese Medicine（TCM） groups （A）with Anyou granule （consisting of rhubarb,root of herbaceouspeong,Areca catechu L,red sage root,copitis thizome,netgrass fiatsedge rhizome） ;the western medicine group （B） with Clarithromycin, Losec;and the combined treated group （C）with Anyou grenule and Clarithromycin, Losec in combination. The efficacy was evaluated after four weeks of treatment. Results： The clinic clearance of H. pylori in the three groups was 38.24% in the A 15.13% in the B,and66.67% in the C： resfactively, showing diffierence of statistical significance between the A and B （ P 〈 0.05 ） ;significant diffierence in the A,B and C. In the DOB value of C^13 -breath test,significant difference was shown between B and C（ P 〈 0.01 ）. Also between A and C（ P 〈0.05）and shown diffience of statistical significence between B and C （ P 〈 0.05 ） Conclusion： Combination of treatment of traditional Chinese medicine and westren medicine call decreased the DOB value of C^13. Breath best evidently in children＇ s H. pylori gastropathy of accumulation symptom of heat in middle -jiao ,and can increase the clinical clearance of H. pylori.
