左旋卡尼汀在维持性血液透析患者中的应用观察

摘　　要：目的观察左旋卡尼汀对维持性血液透析患者的治疗作用及不良反应。方法将50例维持性血液透析患者随机分为2组,每组各25例,治疗组每次血液透析后给予左卡尼汀1.0 g静脉注射,对照组静脉注射等量生理盐水,疗程为12周,观察治疗前后血红蛋白(Hb)、红细胞压积(Hct)、血浆白蛋白(Alb)及血脂水平的变化。结果治疗组患者的血红蛋白、红细胞压积、血浆白蛋白上升的水平显著高于对照组,且甘油三酯下降。不良反应为恶心、呕吐及皮疹。结论左旋卡尼汀治疗安全有效,可以改善血液透析患者营养状态及肾性贫血,降低血脂。Objective To investigate the effect and safety of intravenous infusion of L-carnitine on maintenance hemodialysis patients. Methods Fifty maintenance hemodialysis patients were randomly divided into 2 groups： a treatment group （n=25） and a control group （n=25）. In the treatment group, 1.0 g of L-carnitine was infused to the patients at the end of each dialysis treatment for 12 weeks, while normal saline was given to the control group. Hb, Hct, Alb and blood lipid were determined. Results Hb, Hct and Alb increased significantly, blood lipid decreased significantly in the treatment group. And side reaction was naupathia, vomiting and tetter. Conclusion Intravenous supplement of L-carnitine can improve nutrition and uremic anemia in hemodialysis patients effectively
