摘　　要：目的评价腰椎小关节腔注射曲安奈德治疗腰背痛的效果。方法选择腰椎小关节性腰背病患者80例，随机将其分为小关节腔组（Ⅰ组）与硬膜外腔组（Ⅱ组），每组各40例。Ⅰ组腰椎关节腔注入曲安奈德40mg、0．5％布比卡因5ml。Ⅱ组硬膜外腔注入曲安奈德40mg、2％利多卡因2ml、0．9％生理盐水7ml。观察两组患者的镇痛效果。结果Ⅰ组镇痛优良率95％，Ⅱ组镇痛优良率82．5％，两组比较差异有统计学意义（P〈0．05）。Ⅰ组疼痛缓解持续时间12个月以上52．5％，与Ⅱ组（12．5％）比较差异有统计学意义（P〈0．01）。结论腰椎小关节腔注射曲安奈德治疗腰背痛镇痛效果好，疼痛缓解持续时间长。Objective To evaluate effect of triamcinolone using intra - articular lumbar facet joint injection for the lumbar - back pain. Methods Eighty patients who were diagnosed the lumbar facet joint diseases were randomly divided into the facet joint space group（ group Ⅰ ） and the epidural space group（ group Ⅱ ） ,forty cases per group. Triamcinolone 40 mg,0.5% bupivacaine 0.5 ml and 10% GS 0.5 ml were injected into the lumbar facet joint space in the group Ⅰ. Triamcinolone 40 mg,2% lidocaine 2 ml and 0.9% NS 7 ml were injected into the epidural space in the group Ⅱ, We observed the analgesic effects of these two groups. Results The ratio of excellent analgesic effects was 82. 5 % in the group Ⅰ and 95 % in the group Ⅱ , there had significant difference in two groups（ P 〈 0.05 ）. The duration of pain mitigation being over 12 months was 52.5% ,there had significant difference compared with that of group Ⅱ （ 12. 5 % ） （ P 〈 0.01 ）. Conclusion The analgesic effects of lumbar - back pain is good using triameinolone injected into the lumbar facet joint space,and the duration of pain mitigation is long.
