摘 要:目的分析铜绿假单胞菌纸片扩散药敏试验中抑菌环内出现菌落原因,并为其进行耐药分析,使结果正确解释提供依据。方法采用K—B法进行药物敏感试验,筛选出抑菌环内出现的菌落,进行重复细菌鉴定药敏试验,通过改良三维试验法对其所产β-内酰胺酶进行分析。结果在65株铜绿假单胞菌中产ESBLs有15株(23.1%),26株高产AmpC(40.0%),14株(21.5%)为SSBLsβ-内酰胺酶,1株(1.5%)显示碳青酶烯酶活性,9株(13.8%)未检测出任何活性酶。结论铜绿假单胞菌纸片扩散药敏试验中β-内酰胺类抗生素抑菌环内细胞是其失去抑制产生突变,产ESBLs和(或)高产AmpC变异予个体。医院感染中铜绿假单胞菌抑菌环内出现的菌落现象,在排除污染后对其结果应解释为耐药。Objective To elucidate the reasons for presentation of small colonies in inhibitory zone in K - B diffusion test of Pseudomonas aeruginosa, and then analyse their drug resistance, thus to provide a basis for correct interpretation of susceptibility for pseudomonas aeruginosa. Methods At first,the small colonies of Pseudomonas aeruginosa were selected with K - B susceptibility method, and repeated antimicrobial sensitivity tests, then the modified three dimensional method was taken to analyse the beta - lactamases. Results From 65 strains pseudomonas aeruginosa, 15 strains produced β - lactanase ( ESBLs ) ( 23. 1% ) ; 26 strains produced high level AmpC bera - lactamases (40. 5% ) ; 14 strains produced SSBLs (21.5%) ; One strain ( 1.5% ) produced carbapenemase;and 9 strains( 13.8% ) didn' t produce any enzyme. Conclusion Zoners in the inhibitory zone of beta - lactamases are beta- lactamase cells and(or) AmpC beta- lactamases high produced cells resulting from depressed mutation. The emergence of zoners in the inhibitory zone of Pseudomonas aeruginosa from nosocomial infectionals should be considered as resistance under the circumstance of pre - excluding of contamination.
