摘  要:目的探讨非感染状态下,腹透液(PDF)对腹膜间皮细胞(PMCs)产生单核细胞趋化蛋白-1(MCP-1)的影响,及其与持续性不卧床腹膜透析(CAPD)相关性腹膜硬化的关系。方法将40只大鼠随机分为5组,除正常对照组外分别给予1.5%、2.5%和4.25%PDF及NS腹腔注射,40ml/d,第15天取标本检测PMCs中MCP-1mRNA表达,血清和PDF中MCP-1浓度,腹膜纤维连接蛋白(FN)的表达。结果3个腹透液实验组PMCs中MCP-1mRNA表达,4.25%腹透液组高于2.5%组和1.5%组,差异具有显著性(P<0.05);PDF中MCP-1水平和腹膜中FN表达,4.25%腹透液组最高,但与2.5%和1.5%组比较差异没有显著性。血清中的MCP-1水平在5组中未见显著差异(P>0.05),且均低于PDF中。FN水平与PMCs中MCP-1mRNA表达和PDF中MCP-1蛋白质水平均呈正相关。结论高浓度PDF可使PMCs中MCP-1mRNA表达升高。MCP-1升高可能是CAPD相关性腹膜硬化的原因之一。大量NS灌入腹腔也可导致腹腔炎症状态。Objective To investigate the effect of PDF on MCP-1 produced by PMC in non-infection situations and the associations with CAPD related peritoneal sclerosis. Methods The study was performed on 40 male SD rats ,which were randomly divided into 5 groups ( n = 8 ) and injected intraperitoneally with 40 ml 1.5% , 2.5% , 4.25% PDF and NS daily for 2 weeks,and the 5th group gave no solution as control. On the 15th day, after a 4-hour dwell, the rats were put to death, dialysate samples and serum were collected to measure the MCP-1 protein levels using an ELISA kit,peritoneum tissue were taken to characterize the MCP-1 mRNA expression by ISH and the FN accumulation by ICH. Results In the 3 PDF experimental groups, the MCP-1 mRNA expression of 4.25% group was higher than those of 2.5% group and 1.5% group, and the differences were statistically significant (P 〈0.05). The MCP-1 protein levels of PDF and the FN levels of parietal peritoneal tissue were increased in 4.25% group compared with those of 1.5% and 2.5% groups, but the difference had no statistic significance. The MCP-1 protein levels of serum in the whole 5 groups showed no significant difference ( P 〉 0.05 ), and the values of them were lower than that of PDF respectively. The correlation analysis between FN and MCP-1 of PDF, and FN and MCP-1 mRNA showed positive correlation. Conclusion The study confirmed that high glucose concentration PDF(4.25% ) caused the higher level of MCP-1 mRNA expression of PMCs. We can conclude that the increased expression of MCP-1 may be one of the reasons inducing CAPD related peritoneal sclerosis. A large amount of NS injecte intraperitoneally may also inducing preinflammatory situation of peritoneal cavity.
