摘　　要：目的总结后路经椎弓根固定治疗胸腰椎骨折的治疗经验,评估其临床疗效。方法采用后路经椎弓根内固定治疗胸腰椎骨折19例,按McAfee分类,楔形压缩骨折5例,稳定爆裂性骨折5例,不稳定爆裂性骨折9例。术前术后X线片,CT片测量椎体高度恢复率,椎管横切面积及Cobb's角变化。结果所有病例均获随访,随访平均时间12月。伤椎前后缘高度和Cobb's角分别是术前38.6%、83.1%、35.4°,术后92.7%、98.4%、4.5°,椎管截面积术前平均43.7%,术后92.6%(P<0.01)。结论应用后路椎弓根内固定治疗胸腰段椎体骨折可以获得满意疗效。Aim To sum up the treatment experience and evaluate the effect of posterior transpedicle fixation for thoracolumbar fractures.Methods 19 cases of thoracolumbar fractures treated with transpedicle fixation were analyzed.According to McAfee classification,there were 5 cases of wedge compression fractures,5 stable burst fractures,9 unstable burst fractures.The vertebral body high,spinal canal diameter in sagittal,transverse planes and Cobb＇s angle were obtained.Results All cases were followed up for 12 months on average.The average anterior and posterior height of fractured vertebrae was 38.65%,83.1% preoperatively and 92.7%,98.4% postoperatively;Cobb＇s was 35.4° preoperatively and 4.5° postoperatively.The cross-section area of spinal canal was 43.7% preoperatively,92.6% postoperatively.There was significant difference in targets between pre-operation and post-operation（P〈0.01）.Conclusion Satisfactory outcome can be obtained in the treatment of thoracolumbar fractures with posterior transpedicle screws fixation.
