以VEGF、MMP-2检测验证幽门螺杆菌L型感染与胃癌的关系

摘　　要：目的研究幽门螺杆菌L型(Helicobacter pyloriL-form,Hp-L型)感染对胃癌的发生、发展和侵袭、转移等生物学行为的影响,并探讨其机制。方法应用革兰染色和免疫组化SP法检测130例胃癌和50例对照组的Hp-L型感染,应用免疫组化SP法检测上述组织的血管内皮生长因子(vascular endothelial growth factor,VEGF)、基质金属蛋白酶-2(matrixmet-alloproteinase-2,MMP-2)蛋白的表达。结果130例胃癌中免疫组化及革兰染色同时Hp-L型阳性的病例有88例。胃癌组与对照组的Hp-L型阳性率相比具有统计学意义(67.69%vs24%,P<0.05)。胃癌组中Hp-L型阳性组的MMP-2、VEGF表达阳性率高于其Hp-L型阴性组(P<0.05);胃癌组中Hp-L型阳性MMP-2和VEGF的表达阳性呈正相关(r=0.45,r=0.30,P<0.001)。结论Hp-L型感染是影响胃癌侵袭和转移的重要因素之一。其机制与胃癌细胞的VEGF、MMP-2的表达上调有关。To investigate the correlation between infection with L-form of Helicobacter pylori （Hp-L） and the biological behaviors of gastric cancer, such as its development, tumorigenesis, invasion and metastasis, Hp-L was examined in 130 cases of gastric carcinoma and 50 healthy individuals by means of Gram stain and immunohistochemieal staining with SP method. In addition, the expressions of vascular endothelial growth factor（VEGF） and matrix-metalloproteinase-2（MMP-2）were detected in these 130 cases with gastric carcinoma by the immunohistoehemieal staining with SP method. It was found that in these cases with gasric carcinoma, 88 cases（67.69 %） appeared to be positive for Hp-L as revealed both in Gram＇s stain and the immunohistochemieal staining with SP method; while in the normal controls , the positive rate for Hp-L was only 24 %. （P〈0.05） Also, the percentage of the expression in VEGF and MMP-2 proteins was higher in case of patients with gastric carcinoma in comparison with those of the Hp-L-negative cases（P〈0.05）. Positive correlation existed between positive Hp-L stainings and the expressions of VEGF and MMP-2（r=0.45,r=0.30, P〈0. 001）. It seems that infection with L-form of H. pylori may be an important factor influencing the invasion and metastasis of gastric carcinoma and this may be related to the up-regulated expressions of VEGF and MMP-2.
