脓毒症患者血磷浓度变化对疾病预后的影响

摘　　要：目的研究脓毒症患者血磷水平变化及其与脓毒症患者预后的关系。方法回顾性分析82例脓毒症患者血磷水平变化,及其与急性生理学及慢性健康状况评分(APACHEⅡ)和预后的关系。结果脓毒症组血磷水平[(0.79±0.27)mmol/L]较对照组[(1.04±0.43)mmol/L]低(t=3.296,P<0.01);正常血磷组与低磷血症组死亡率比较差异有统计学意义(χ2=4.457,P<0.05);存活组与死亡组血磷低于正常值的标本数差异有统计学意义(χ2=8.483,P<0.01);血磷水平与APACHEⅡ评分值呈负相关(r=-0.239,P<0.01)。结论脓毒症患者易出现低磷血症,并且血磷水平低者,预后较差,血磷水平可作为判断脓毒症患者预后的简易指标之一。Objective To study the concentration of serum phosphate in patients with sepsis,and the relationship between the serum phosphate concentration and the prognosis of patients with sepsis. Methods 82 patients with sepsis in ICU were enrolled. Acute physiology and chronic health evaluation Ⅱ （ APACHE Ⅱ ） score and the concentration of serum phosphate were determined in the patients. AU eases were divided into （ 1 ） the normal phosphate concentration and the hypophosphatemia group. Results The concentration of serum phosphate was lower in sepsis group than that in control group （t = 3. 296,P 〈 0. 01 ）, the mortality of the hypophosphatemia group was higher than that of normal phosphate group （X^2 = 4.457, P 〈 0. 05 ）. The incidence rate of hypophesphatemia in death group was higher than that in survied group （ X^2 = 8. 483 ,P 〈 0. 01 ）. The concentration of serum phosphate was negative correlation with the APACHE Ⅱ score （ r = - 0.239,P 〈 0. 01 ）. Conclusion The concentration of serum phosphate is correlated to the prognosis of patients with sepsis; and could be used as an index in judging the outcome of patients with sepsis.
