建立慢性湿疹中医生存质量量表的初步研究

摘　　要：目的:研制具有中医内涵的慢性湿疹患者生存质量量表,全面、客观的评价中医药治疗慢性湿疹的临床疗效。方法:根据WHO生存质量量表制定原则,结合中医基础理论,提出量表的理论构想,广泛查阅国内外多种普遍性及湿疹专用生存质量量表,采用结构化决策的方法,构建初始量表。用调查方式对120名慢性湿疹患者进行了测试,以离散趋势、相关分析、因子分析、等5种方法多维度筛选条目,最终形成慢性湿疹生存质量量表(The Quality of Life Scale for Chronic Eczema Patients-Prior TestVersion,EQOLS)。结果:研制出生理、心理、社会3个维度共26个条目组成EQOLS,与理论构想相符。EQOLS 24 h重测信度为0.986,Cronbach’α系数为0.880,分半信度系数为0.881。因子分析提取了5个公因式,累计方差贡献率73.027%,第一公因式为湿疹对日常生活能力的影响,方差贡献率48.483%。各维度与总分相关系数于0.710-0.894之间。EQOLS总分与SF-36总分、DLQI、EASI总分相关系数分别为0.779、0.762、0.660。患者中医药治疗前后EQOLS总分差异有统计学意义,不同病情组、不同病程组、不同年龄组、不同中医辨证分型组的患者EQOLS总分差异均有统计学意义。结论:EQOLS是可靠、有效、灵敏的,同时具有中医特色,准确反映了慢性湿疹患者生存质量的内涵,可作为慢性湿疹中医临床疗效评价的工具。Objective： To develop quality of life（QOL） scale for chornic eczema patients with Traditional Chinese Mendicine（TCM） feature and evaluate curative effects of impersonally. Methods：The theory construction was put forward on the base of quality of llfe definetion from WHO and theory of TCM. Generic and eczema-specific quality of life scales had been extensively consulted and methods adhered to rigorous guidelines of instrument had been adapted before the item pool formed. With the methods of investigation, One hundred and twenty patients of chronic eczema had been reeruite in the study.Five methods had been used in item analysis,such as methods of analysis, factor analysis and so on.Item reserved form the Quality of Life Scale for Chronic Eczema Patients-Prior Test Version（EQOLS）.The reliability and validity of the scale were also tested. Result：An 26-items Quality of Life Scale for Eczema Patiens（EQOLS）, include 3 domains（physical,social, psychological）was developed. It was consistent with theory construction. The test-retest correlation coefficient, Cronbach＇s α coefficient and split-half reliability coefficient of EQOLS were 0.986,0.880,0.881.5 common factors were estracted and cumulative variance was 73.027%. The largest common factor who explained 48.483% of total variance was part of physical domain.The domain-total correlationlls of EQOLS were 0.710-0.894.The scale＇s correlation with SF-36,DLQI,EASI score were 0.779, 0.762, 0.660 respectively. EQOLS can well discriminate among those with different quality of lives. Conclusion： EQOLS is reliable,valid and sensitive.The scale with TCM feature interpretes the connotation of chronic eczema＇quallty of accurately.It can be used as means of clinical curative effect with TCM.
