彩色多普勒超声对颈部结节病性淋巴结炎的诊断价值

摘　　要：目的探讨彩色多普勒超声对颈部结节病性淋巴结炎的诊断。方法用彩色多普勒超声观察与测量颈部结节病性淋巴结炎36例,结核性肿大淋巴结34例及恶性淋巴瘤32例的长径与前后径的比值(L/S),内部回声,彩色多普勒血流图(CDFI),血流分布与脉冲多普勒(PWD)频谱特征。结果结节病性淋巴结炎L/S的比值平均为2.0±0.61,阻力指数(RI)为0.53±0.08,血流分布较规则,呈"树枝"状;结核性肿大淋巴结L/S的比值平均为2.6±0.29,RI为0.49±0.22,血流分布较少;恶性淋巴瘤L/S的比值平均为1.5±0.36,RI为0.72±0.11,血流分布不规则,紊乱。3项指标比较差异有统计学意义(P<0.05)。Vmax、Vmin组间比较差异无统计学意义(P>0.05)。结论结合胸片及临床表现,超声的声像图对结节性淋巴结炎具有一定的诊断价值。Objective To explore the diagnostic value of color Doppler tdtrasound in in neck lymphadenitis of sarcoidosis.Methods The L/S ratio, internal echo, CDFI, the characteristics of flow shape, distribution and RI frequency spectnun feature were measured respectively with color doppler tdtrasound in 36 cases of sarcoidosis lymph nodes, 34 cases of tuberetdous lymph nodes and 32 cases of malignant lymph nodes located around neck. Results The average L/S rotio and Resistant Index （RI） of sarcoidosis lymph node group were 2.0 ± 0.61 and 0.53 ± 0.08 respectively, and the blood distribution of the group showed ＂branch＂ shape blood flow, regular formation. The average L/S rotio and RI of tuberetdous lymph node group were 2.6 ± 0.29 and 0.49 ± 0.22 respectively, and the blood distribution of the group showed very small spectrum. The average L/S rotio and RI of maligant lymph node group were 1.5 ± 0.36 and 0.72 ± 0.11 respectively , and the blood distribution of the group showed deformation disorder spectrum. There were significant differences in the three groups （ P 〈 0.05 ）. Conclusion Combined with chest X-ray and clinical findings, the color doppler tdtrasound has important diagnostic value for neck lymphadenitis of sarcoidosis.
