摘　　要：目的评价圆锥形钛质听骨在鼓室成形手术中的应用效果。方法对我院32例慢性化脓性中耳炎患者,在接受人工听骨部分赝复物——圆锥形钛质听骨植入及听骨链重建术后进行随访。结果30耳鼓膜修补后1个月复查均愈合良好,但术后4个月时发现2例听骨脱出。30耳术前气导听力(58.02±12.79)dB,术后气导听力平均为(35.03±14.23)dB,术后听力较术前明显提高(P<0.01)。术前骨导听力(19.22±11.33)dB,术后骨导听力(22.28±11.11)dB,术前、术后听力无明显变化(P>0.05)。术前气骨导差(ABG)(38.71±11.19)dB;术后气骨导差(ABG)(20.92±12.03)dB,与术前相比气骨导差下降明显(P<0.01)。术后听力提高总有效率为76.7%。结论锥形钛质听骨应用于中耳乳突手术中可以获得良好的听力效果,它是一种并发症少,值得推广应用的人工听骨材料。Objective To evaluate the effects of ossicular reconstruction with taper titanic ossicular replacement prosthesis.Methods The efficacy of ossicular reconstruction with taper titanic ossicular replacement prosthesis was retrospectively summed up in 32 patients who were followed up after tympanoplasty.Results 30 nepaired tympanie membranes healed up after 1 menth when followed up,however 2 ossicular replace ment prosthesis pushed out after 4 months.The mean air conduction hearing thneshold was(58.02±12.79)dB before opreation,while it was(35.03±14.23)dB after operation(P<0.01).The mean bone conduction hearing threshold was(19.22±11.33)dB before operation,while it was(22.28±11.11)dB after operation(P>0.05).The mean air bone gap(ABG)of 30 ears was(38.71±11.19)dB before operation,while it was elevated to(20.92±12.03)dB when followed up(P<0.01).The total effectiveness was 76.7%.Conclusion taper titanic ossicular replacement prosthesis could be a useful material for ossicular reconstruction and hearing improvement was obtained after operation for otitis media patients.
