改良式经皮扩张气管切开术与传统式外科开放性气管切开术的对比研究

摘　　要：目的比较改良式经皮扩张气管切开术(modified percutaneous dilative tracheostomy,MPDT)和传统式外科开放性气管切开术(operational tracheostomy,OT)的实用性及特点。方法将65例需机械通气ICU患者采用不同两种方法进行气管切开。其中MPDT组34例,OT组31例,观察两组的手术切口、手术时间、出血、手术成本费用及并发症。结果MPDT组的手术切口长度、手术时间、术中、术后出血量明显<OT组(P<0.05),而手术成本费用及其它并发症无明显区别(P>0.05)。结论MPDT具有安全、简单、快速、损伤小的特点、与经皮扩张气管切开术(PDT)性能相当,而手术材料费用却明显降低,在患者急救中具有推广应用价值。Objective Compare the practicability and characteristics between modified percutaneous dilative tracheostomy （MPDT） and traditional operational tracheostomy （OT）. Methods sixty-five ICU cases needed mechanical ventilation were divided into two groups, MPDT group thirth-four cases and OT group thirty-one cases. The length of incision wound, duration of operation, the amount of haemorrage,cost and complications were observed respectively in two groups. Results In MPDT group,the length of incision wound, duration of operation, the amount of haemorrage were significantly less than the OT group（ P 〈 0.05 ）, but the cost and other complications have no differece（ P 〉 0.05 ）. Conclusion MPDT method is safer, more convenient, less traumatic, and less duration of operation than OT. And its cost is lower than PDT. It can be widely used in the treatment of emergency patients.
