高氧液治疗Ⅰ型呼吸衰竭的疗效评价

摘　　要：目的：观察静脉输注高氧液对Ⅰ型呼吸衰竭患者氧合和血液流变学指标的影响及入院后28dMODS的发生率和预后（存活或死亡），从而评价其临床应用价值。方法：将入选患者60例，随机分成试验组和对照组各30例。对照组予气道给氧，试验组在气道给氧的基础上。加用静脉给氧。观察治疗前、治疗后2h、3d、7d动脉血气、血液流变学指标及入院后28d MODS的发生率和预后（存活或死亡）。结果：①试验组患者脉搏氧饱和度（SpO2）、动脉血氧饱和度（SaO2）明显上升；②2h、3d、7d时点与同组0h点比较有显著性差别（P〈0．05），与对照组比较也有显著性差别（P〈0．05）；③试验组全血黏度下降（P〈0．05）；④入院后28d MODS的发生率和预后两组有显著性差别（P〈0．05）。结论：高氧液对Ⅰ型呼吸衰竭患者的治疗是有益的。Objective：To examine the impact of hyperoxygenated solution infusion via vein on oxygenation and hemorheology indicators of patients with type I respiratory failure and the incidence and prognosis （ survival or death） of MODS at 28 days after hospitalized for evaluating its clinical application. Methods：60 patients were randomly divided into two groups of 30 each. The control group were supplied oxygen via airway, and the experiment group were supplied oxygen via vein on the basis of airway oxygenation. The arterial blood gas, hemorheology indicators were observed before and after treatment for 2h, 3 d, 7 d. Also the incidence and prognosis of MODS were observed at 28 days after hospitalized. Results： ①The SpO2 and SaO2 of patients from experiment group increased significantly;②The parameters at 2 hour,3 days and 7 days had significant difference compared with 0 hour, and also had significant difference compared with control group at the same point（P 〈 0. 05 ） ;③The whole blood viscosity of experiment group decreased（ P 〈 0.05 ） ;④The incidence and prognosis of MODS at 28 days after hospitalized had significant difference between experiment group and control group （ P 〈 0.05 ）. Conclusion ： It is helpful that hyperoxygenated solution treatment the patients of typeⅠ.
