肱骨近端锁定钢板治疗粉碎性肱骨近端骨折

摘　　要：目的：探讨锁定钢板治疗肱骨近端粉碎性骨折的疗效。方法：38例肱骨近端粉碎性骨折行切开复位钢板内固定术。其中18例用肱骨近端锁定钢板固定（A组），另20例用三叶草钢板固定（B组）。结果：全部病例得到随访（12～24个月），平均15个月。A组骨折全部愈合，愈合时时间平均3个月，肩关节活动良好，没有发生不愈合或延迟愈合及肱骨头缺血性坏死。B组骨折也全部愈合，愈合时间平均4．5个月，1例发生螺钉松动，1例术后骨折有移位，1例钢板断裂合并延迟愈合，无肱骨头缺血性坏死。结论：用肱骨近端锁定钢板治疗粉碎性肱骨近端骨折具有固定牢靠、骨折愈合快、功能恢复良好的优点。Objective： To discuss the therapeutic effect of comminuted fracture of proximal humerus with locking proximal humerus plate （LPHP）. Methods：Treating 38 cases of comminuted fracture of proximal humerus via open reduction 18 patients were treated by LPHP （Group A）, while 20 by three leaves plate （Group B）. Results： All patients have already been followed up：And the follow- up period from 12 months to 24 months,averagely 15 months. All patients in Group A had been union and their shoulders had good functions without humeral head avascular necrosis, nonunion or put off union. And the union period is 3 months on average. All patients in Group B had also been union with average union period of 4. 5 months. Among those patients,2 patients＇ screws loosed, 1 patient＇s fracture was dislocated after surgery, and 1 patient＇s plate cracked, which led to put off union. However, there＇s no humeral head necrosis. Conclusion：It＇s fixed, steady, rapid recover of fracture and sound recover of functioin to treat comminuted fracture of proximal humerus with LPHP.
