体外循环围手术期凝血功能及血小板数量变化的临床观察

摘　　要：目的:研究体外循环术期间凝血功能、抗凝血功能、纤溶系统活性和血小板数量的改变。方法:测定35例进行心脏手术患者体外循环术前、中、后的凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)、纤维蛋白原(FIB)、血小板(PLT)、抗凝血酶(AT)和D二聚体(D-Dimer)指标。结果:体外循环术中和术后凝血指标PT、APTT、TT与体外循环术前相比均明显延长,P<0.01。FIB、PLT、D-Dimer体外循环术中和术后与体外循环术前相比均降低,P<0.01或P<0.05。结论:体外循环术可以导致凝血、抗凝血、纤溶系统活性和血小板数量的改变,引起术后非外科性出血。Objective To observe changes of coagulant function and platelet quantity during perioperative extracorporeal circulation. Method The levels of PT,APTT,TT,FIB,AT,PLT and D-Dimer were detected in 35 perioperative patients,who underwent intracardiac surgery under extracorporeal circulation. Results The levels of PT, APTT and TT during and after operation were obviously longer than those before operation （ P 〈 0. 01 ）. However, the levels of FIB, PLT and D-Dimer during and after operation were obviously lower than those before operation（ P 〈0.05 or P 〈0.01 ）. Conelusion The extracorporeal circulation operation might effect the function of coagulant, anticoagulant,fibrinolytic system and platelet quantity, which induce non-surgical hemorrhage after operation.
