肾实质小切口联合液电碎石术治疗复杂性肾结石的疗效观察

摘　　要：目的探讨肾实质小切口联合液电碎石术治疗复杂性肾鹿角状或铸型结石的疗效。方法对1988年8月—2006年10月沈阳军区泌尿外科中心收治的272例肾窦内肾盂肾鹿角状或铸型结石患者行肾实质小切口联合液电碎石术的临床资料进行总结分析。结果手术平均耗时146min,术中输血15例,输血400-600ml。术后平均住院14.5d。259例结石一次性取净,残余结石13例,直径〈0.6cm。术后3周行泌尿系平片及静脉泌尿系造影或B型超声检查,肾脏显影正常,无明显积水。结论肾实质小切口联合液电碎石术治疗复杂性肾结石具有损伤小,出血少、安全、结石取净率高、手术相对简单、术后恢复快等优点,是治疗复杂性肾结石的有效方法之一。Objective To investigate the effects of renal parenchyma discussion with miniincision in combination with electrohydraulic lithotripsy for the treatment of complicated staghorn-like calculi. Methods The clinical data of 272 cases of complicated staghorn calculus （from August 1988 to October 2006） were retrospectively analyzed. All the patients, who were selected from the Urological Surgery Center, Shenyang Military Region, underwent renal parenchyma discussion with mini-incision in combination with electrohvdraulic lithotripsy.Results The mean operative time was 146 minutes （intraoperative blood transfusion from 400 to 600 ml was needed in 15 patients）, and the mean duration of postoperative hospitalization was 14.5 days. The calculi were completely cleared out in 259 cases, and the residual calculi （diameter〈0.6 cm） existed in 13 cases. No abnormal findings were found in the examinations such as B-ultrasonic detection three weeks later after operation.Conclusion Combination of renal parenchyma discission with mini-incision and electrohydraulic 1ithotripsy proves a good meathod to the treatment of complicated renal calculi because of its advantage such as less damage, less blood loss, high safty, high stone clearance rate, easy operation and rapid postoperative recovery.
