摘　　要：目的介绍改良Rives—Stoppa修补术治疗腹壁巨大切口疝的方法和效果。方法腹壁巨大切口疝26例，腹壁缺损平均直径14．5cm，均行改良Rives—Stoppa修补术，将聚丙烯补片置于腹直肌后筋膜前间隙，补片超过缺损至少5cm，固定补片缝线均在腹直肌前鞘前结扎。结果术后4例出现皮下积液，经穿刺抽吸治愈。26例术后均痊愈出院，无手术死亡，无切口感染和补片感染；随访随访3月～5年，无复发病例。结论改良Rives—Stoppa修补术是治疗腹壁巨大切口疝的理想手术。Objective To introduce the procedure of modified Rives-Stoppa operation for repairing the giant incisional hernia and evaluate its efficacy. Methods 26 patients with giant incisional hernia were received modified Rives-Stoppa operation. The mean diameter of the abdominal wall defect was 14.5 cm. Polypropylene mesh was placed in front of the posterior sheath of rectus abdominis. The patch covered over the defect at least 5 cm, and the fixed patch sutures were tied before the anterior sheath. Results No operative death, nor wound or mesh infection occurred postoperatively. But subcutaneous hydrops were found in 4 cases who were cured by puncture and aspiration. All cases were followed up without recurrence for 3 months to 5 years. Conclusion The modified Rives-Stoppa procedure is an ideal repair method for giant incisional hernia.
